      • Push pins observed on walls of younger toddler classroom.-Please remove the push pins and replace with protective material to attached items on the walls. This is to prevent or reduce children's access to hazardous materials.Additionally, please cover staple pins observed on the walls with adhesive tape.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Torn diaper mats were observed in baby and younger babies classrooms.-Please replace the diaper mats to ensure proper cleaning and sanitizing of the surfaces.

      • Worn chair observed in the Before and After School classroom.-Please repair or replace the indicated chair to ensure effective cleaning and sanitizing of surfaces.

      • Hot water temperature was measured between 55 and 57'C in baby and 3- & 4-years old classrooms.-Please lower and maintain hot water temperature between 38 and 43'C to prevent the risk of scalding.

      • An uncovered light fixture was observed in the Before and After School classroom.-Please install a suitable cover for the identified light fixture or replace the existing bulbs with shatter-resistant (non-shatter) types.
    • 06. Are proper protocols being followed in regards to client care?
      • Sanitizer concentration was measured at 10 ppm in the 3- and 4-year-old classroom.Required Action:Please increase and maintain the bleach solution concentration at 100 ppm to ensure effective cleaning and sanitizing of classroom surfaces and equipment.Additionally, please ensure that the bleach concentration is verified daily using a suitable test strip.

      • Paper towels kept in open basket was observed in toddler/baby room.-Please provide suitable paper towel dispenser for the indicated room to prevent bulk contamination.

      • Worn chair observed in the Before and After School classroom.-Please repair or replace the indicated chair to ensure effective cleaning and sanitizing of surfaces.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Climbable elevated play structure with 8 feet 3 inches fall height was observed without adequate fall zone surfacing.-Please provide adequate fall zone surfacing for the indicated structure.

      • 1. Hot water temperature was measured between 55 and 57'C in baby and 3- & 4-years old classrooms.-Please lower and maintain hot water temperature between 38 and 43'C to prevent the risk of scalding.2. Faulty faucet noted in older toddler classroom.-Please fix and maintain good repair.

      • Last inspection, it was noted: "The child accessible handwashing sink in the newly proposed space had a temperature exceeding 54C at the maximum (approximately 58C at the time of the inspection)."This inspection, the water temperature was measured to be approximately 62C at the new site. The other handwashing taps had temperatures approximately 56C.REQUIREMENT: Lower the maximum temperature at this handwashing station so it does not exceed the recommended temperature of 49C.

      • Staples were being used to support wall art and posters in the infant room.REQUIREMENT: Remove the staples and do not use staples to support posting, wall art, etc. in the infant room; use tape instead.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kits in several rooms were not properly supplied and some products were expired (e.g., antiseptic wipes since 2016). The inspector provided the list of required items to the Director during the inspection.
