- Non cleanable porous single use tools we're being washed and reused in the facility.**Do not reuse single use tools.**Discard single use tools after each use as items cannot be adequately washed and disinfected.

- 1. Dirty semi-critical tools were observed stored in the cabinet containing clean tools. Staff on site stated tools had been cleaned and disinfected. **Tools are not being adequately cleaned prior to disinfection**Ensure that tools are properly washed and inspected prior to disinfection**Do not store dirty tools with clean tools2. The bowl used to store clean tools in the clean tools' cabinet was dirty.**Please clean storage bowl**Ensure that disinfected tools are stored in a clean and sanitary manner to prevent contamination of tools.

- 1. Incomplete documentation of the daily testing of the reused High-Level Disinfectant was observed during the inspection. Record showed that disinfectant was last tested on February 25th, 2025**Ensure that high level disinfectants that are being reused are tested daily and documented appropriately. **Discard solution according to manufacturer’s instructions.*Copy of HLD log sheet was emailed to operator.2. Improper disinfection of semi critical tools was noted during the inspection. Semi-critical tools in the disinfectant solution were not fully submerged in the solution.**Ensure that tools in the disinfectant solution are fully submerged in the disinfectant for the duration of the contact time as per manufacturer’s instructions.3. Drill bits were noted soaking in alcohol at the manicure station.**Alcohol is not a disinfectant.**Non-critical tools (drill bits) must be disinfected using at least a low-level disinfectant.

- Nail dust was noted around the table and storage cupboards of the manicure station.**Ensure that the manicure station is adequately cleaned frequently to prevent build-up of nail dust around the station.

- Rusty semi-critical tools were noted to be stored in the clean tool's cabinet. Rusty tools were removed.**Ensure that all tools used in the facility are in good repair.**Rusty tools can cause diseases when in contact with client's mucous membrane, do not use rusty or damaged tools in the facility.

- Some used porous items such as nail buffers, nail files and toe separators were stored in the cupboard next to the reprocessing sink.**Nail buffers, nail files and toe separators are porous items that cannot be washed or re-used, do not reuse these items, discard items after each use.
- 15. Is reusable equipment handled and stored properly (Critical)?
- Used drill bits that were visibly dirty were stored in their packaging container and stored in the cupboards at the manicure station without being adequately cleaned and disinfected prior to storage.**Please ensure that dirty tools are properly cleaned and disinfected after each use prior to storage.**Store dirty tools separately from clean tools.
- 17. Is reusable equipment cleaned properly (Critical)?
- Reusable semi-critical tools were not being properly washed prior to disinfection. During the inspection, PHI observed staff rinsing dirty tools under running water without using soap to wash tools, then wet tools were placed into the disinfection bin.**Ensure that dirty tools are properly washed, rinsed and dried and inspected to ensure that tool has been adequately cleaned prior to placing in disinfection solution.
- 20. Is reusable equipment disinfected properly?
- 1. The bin used for disinfecting semi-critical tools was full. Some of the tools placed in the bin were not fully imersed in the disinfectant solution.**Ensure proper disinfection of semi-critical tools.**Ensure that tools are fully immersed in the solution and left in for the entire contact time following manufacturer's instruction, to ensure tools are adequately disinfected.2. Drill bits were soaked in alcohol at several of the manicure station.**Do not use alcohol for disinfecting tools.**A low-level disinfectant may be used for disinfection of drill bits.3. Dirt and debris was noted in the magnetic jet motor of the pedicure chair.**Ensure that the jet motors are disassembled after each use and adequately washed and disinfected after each use using at least a low-level disinfectant.

- 1. The test strip used to test the high level disinfectant in the facility was expired**Please replace the test strip to ensure accurate reading of the disinfectant concentration.2. There was no records of the daily testing of the reused high-level disinfectant in the facility.**Ensure that reused high-level disinfectant are tested and documented daily to ensure that tools are adequately disinfected after use. **Log sheet sent to operator.

- Some bottles of products used in the pedicure station were unlabeled.**Please label all products used in the facility to identify its content.
- 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
- A large number of used hand gloves were noted piled up under the reprocessing sink in the facility. Staff removed all the hand gloves and placed in garbage can.**Ensure that PPE are disposed of in a clean and sanitary manner.**Maintain the facility in a clean state at all times.
