      • The owner of the personal service business did not submit a notification about the business to the Alberta Health Services. Submitted during the inspection.

      • No written procedures for skin cleansing prior to services, reprocessing sink decontamination and accidental cuts were available.

      • The concentration of the high-level disinfectant (Preempt CS20) was not acceptable when tested in the soaking tray using the PHI's test strip. The disinfectant solution was changed during the inspection.

      • The reprocessing sink was also used for hand washing, and no sink decontamination written procedure was available.
