    • 03. Are clients at this facility being protected from hazards (Critical)?
      • -1. The spray bottle containing bleach mixed with water on the upper floor area was not labeled to indicate it's contents.**Label all bottles containing chemicals with it's contents. 2. In the basement preschooler washroom, there was a powdered household cleaning product with it's label indicating it contains bleach in an unlocked cabinet under the children handsinks.** Relocate the cleaning product away from children or lock the cabinet to prevent children access. 3. A can of spray foam was located the basement preschooler storage cubicle, and it was accessible to children. ** Relocate the chemical product away from children or store the foam in a locked cabinet to prevent access. -
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • -1. Wall mounted mirrors throughout the facility in the rooms ( ex. infant room, preschooler room, and toddler room) were not shatterproof or child safe. ** Replace wall mounted mirrors with shatterproof or child safe mirrors. Or, remove the mirrors completely. 2. There were 2 Calvin Crafting glues found in the preschooler room that did not have the AP label or ASTM label. ** Please remove from the site and replace with glue that has an AP label or ASTM label, as these are indications that the product had been tested safe for children use. -
    • 06. Are proper protocols being followed in regards to client care?
      • -1. There was no designated disinfectant bottle available in the upper floor washroom where diapering is being performed. There was only one disinfectant bottle for the whole top floor ( spanning 2 children rooms and a bathroom ) observed. ** Ensure there is a designated disinfectant spray bottle kept in the washroom where diapers are being changed to encourage proper disinfecting of surfaces between changes. Label the bottle. ** Also ensure all rooms has it's own disinfectant spray bottle so rooms do not have to share one bottle. Label the bottle. 2. There was an accumulation of debris noted under the diapering mat on the change table in the upper washroom. ** Please ensure the diaper change table is properly cleaned/ disinfected. 3. One of the portable booster seats in the Infant Room that was being used to secure children with a tray for eating was in disrepair. The corner edges were frayed off , revealing the supports, and the material was absorbent/ not easy to clean in between use. ** Please remove the booster seat in the infant room and replace with a seat/tray that is in good condition / easy to clean.-
    • 08. Is this facility properly constructed (Non-Critical)?
      • -1. The hanging shelves throughout the site did not have proper edge trims installed, therefore revealing porous raw wood. ** Apply proper edge trims to the shelving units so the area is made smooth, non-absorbent to moisture, and easy to clean. 2. The surface where the diapering mat is placed on the change table in the upper washroom was porous raw wood. **Apply a proper finish on the surface to make the area smooth, non-absorbent to moisture, and easy to clean.-
    • 10. Is a properly supplied first aid kit provided and readily available?
      • -The first aid kit was missing the following items: - Gloves, antiseptic wipes, biohazard bags - Emergency blanket (aluminized, non-stretch polyester) , hand/skin cleaning towelette, antibiotic ointment (topical, single use) - these are new requirements**Equip the first aid kit with these missing contents -
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • -1. The faucet in the kitchen was loose at it's base. ** Secure the faucet so it does not move when used or replace completely. 2. Red and white plumbing lines were openly exposed in the upstairs washroom along the wall / floor. **Install a protective cover to enclose the exposed plumbing lines and maintain smooth, cleanable surfaces. -
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • - There was a green dinosaur stuffed toy in the Infant room that had unremovable electronic compartments and would be damaged if laundered.**Remove the toy from the site as this cannot be properly cleaned/disinfected. -
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • -The hot water temperature at the hand sink in the upstairs infant room measured up to 60 degrees Celsius. There was a children step stool and a diapering mat located beside this handsink. ** Install a mixing valve/ tempering device to reduce the maximum temperature at the handsink as this is scalding temperatures. ** Ensure the maximum hot water temperature measures less than 49 degrees celsius. -
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • -The right toilet in the basement preschooler washroom was plugged and no longer draining. ** Please repair the toilet so it is able to flush waste properly. Properly clean and disinfect the area around the toilet in case water overflows.-
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • -Some of the parent provided bags containing bedding were being stored on the floor in the preschooler room and it was not labeled with the child's name. **Ensure all bags are properly labeled with the child's name. -
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • -In the basement preschooler room, there were children's jackets/ backpacks that were hung up on the wall mounted hooks but were overlapping/ touching. ** Please ensure storage of the children's personal items are separated. Install cubicles or dividers to help with separation of items. -
