      • Chlorine sanitizing solutions were found to be at over 800 ppm. The solutions were diluted and verified to be at 100 ppm using a chlorine test strip.Please ensure that sanitizing solutions are maintained at appropriate concentrations.

      • Bleach sanitizer solution located in room 4 was over 200ppm and bleached out the test strip. A new solution was prepared immediately measuring 100ppm. Ensure all sanitizer solutions are tested daily.

    • 06. Are proper protocols being followed in regards to client care?
      • The diapering surface should only be used for the purposes of diapering and the diapering mattress should be fastened to the surface in someway. This could occur by Velcro. or buckle or some other way.
    • 08. Is this facility properly constructed (Non-Critical)?
      • The third cubby in second toddler room was not anchored to the wall.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • The diapering handwashing sink is being used for other purposes. Ensure that the sink is only used as a handwashing sink only. Dirty items should be placed in a bin and brought to the kitchen for cleaning and sanitizing.
