- There were no written procedures for the facility.
- 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
- Manager and employees showed a clear lack of knowledge and understanding regarding the requirements of the PS Regulation and Standards, as well as a lack of understanding or compliance with manufacturer's instructions. Owner was provided with multiple documents in English and Vietnamese, for all staff to read and sign that they understood. Additional training of staff is required.

- Multiple staff were observed removing the pedicure bath liners after a client's service and spraying the bath with an unknown product in an unlabeled spray bottle. Upon inquiry, this was determined to be alcohol, which is not an approved disinfectant.

- Semi-critical instruments were being left to sit in a large sink. Employees were observed washing their hands in the same sink, overtop of these instruments. CS20 was being used for these instruments, however, it had sat for so long it was no longer light yellow in colour and was instead clear. The records for re-use had not been updated since February. Test strips could not immediately be found, and when they were, they were unopened. Once opened and tested, the product in the instrument soaking container was shown to fail the test for required concentration. The bulk supply was tested and did pass.

- Nail polishes did not have ingredients or manufacturer's instructions for use listed for the following brands; Govandi, Vendeeni, Haux, Danny Coll, Moding, Suseegel.
- 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
- The washroom for clients had a roll of paper towel sitting on the bathroom countertop, not in a dispenser. This was removed at the request of the PHI, as there is a hand dryer available.
