- It was noted that staff members were not washing hands between diapering in E2. Three children were noted in the washroom. Please ensure to train staff with diapering hand washing and only bring one child at a time.

- 05. Are communicable diseases at this facility being properly managed?
- 1. It was noted that the facility was using 100 ppm bleach solution. The operator was advised to use bleach solution at 1000 ppm during outbreak. The operator prepared fresh bleach solution at 1000 ppm during the inspection.2. High touch surfaces were not being disinfected. Please ensure that all high touch surfaces are being sanitized frequently during GI outbreak.

- 1. Items such as dustpan, gloves and cream were noted on the diapering station in the E1 room. Please remove all items from the diapering station and please not use this area for storage purposes. 2. The diapering area was not disinfected after removing dustpan. Please disinfect diapering area properly. 3. It was noted that staff members were not washing hands between diapering in E1. Staff member was noted to be removing dustpan from the diapering station then proceeded to change diaper in the washroom and started touching children without performing hand washing. After mentioning by the PHI and director, the staff member rinsed their hands without performing hand washing using soap. When directed, the staff member used soap but didn't perform hand hygiene properly and rinsed hands right after without rubbing. Staff member also noted to be rinsing gloves.All this was discussed with the operator during the inspection. Please train staff members with proper hand hygiene.4. Soiled dishes and soiled toys were noted to be stored on the countertop of diapering hand washing sink. The written disinfection procedures to decontaminate sink was not being followed in E1. Please do not use this area for storage purposes. Please follow written sink decontaminate procedures.

- 1. General cleaning and organizing is required. Please clean and organize the facility. 2. Soiled cleaning cloths were noted to be stored with toys. These were moved during the inspection. The operator was further advised not to store soiled cleaning cloths with toys. Please store them in the designated soiled cloth storage container.

- Clients' blankets were stored on top of each other in Nav 1. Please be advised to store sleeping supplies properly.

- Staff member jacket was noted to be stored with clients' personal items. Please be advised that clients personal items must not be in physical contact with staff or other clients.

Staff member was noted to be removing dustpan from the diapering station then proceeded to change diaper in the washroom and started touching children without performing hand washing. After mentioning by the PHI and director, the staff member rinsed their hands without performing hand washing using soap. When directed, the staff member used soap but didn't perform hand hygiene properly and rinsed hands right after without rubbing. The staff member was advised that hand washing includes usage of hand washing soap, rubbing for 20 seconds and then rinsing. The operator was advised to train staff members with proper hand washing procedure, and glove policy.

- One of the infants was noted to be touching the diapering sink and licking her hands immediately after. The staff member also sprayed the sink with a bleach solution without wiping it. Infants were noted to be touching this sink. Please install a baby gate to prevent clients access to this sink.

- It was noted that the facility was making their own playdough with flour. The flour was noted to be stored in the storage room floor. Please do not use regular flour to make playdough. Please either purchase playdough or use corn starch or purchase commercial grade flour that is meant for playdough.

- 1. General cleaning is required in both infant rooms. Debris was noted in all toy storage bins, diapering station. Please clean the facility and storage bins thoroughly and ensure they are maintained in a clean and sanitary manner. 2. Soiled mop was noted in one of the infant rooms and clients had access to it. Please ensure that mops are being washed between uses. Please store brooms and mops away from client's access.3. The storage room needs cleaning and organizing. Please clean and organize the storage room.

- 1. The step stool in the washroom of Explorer 1 and 2 was noted to be peeling. 2. Two tables located in the OSC room were noted to be peeling. Please refinish indicated surfaces.

- 1. It was noted that the facility was not disinfecting toys after washing them. Proper toy washing and disinfection procedures were discussed with the operator during the inspection. Please be advised to wash toys with soap and water, rinse with clean water and disinfect using bleach solution. Plese create a written toy washing procedure and schedule for each room.2. Mouth toy bin was not observed in one of infant rooms. Please ensure that all rooms have a mouth toy bin and ensure these toys are being cleaned and disinfected every day.

- 1. It was noted that the facility was using reusable cloth to disinfect sink between diapering and client's handwashing. Please do not use reusable cloth to disinfect the sink area.2. Food items were noted in both infant rooms. The facility does not have a separate hand washing sink for food prep. Please note that all food prep must occur in the kitchen area.

- It was noted that the facility was not testing bleach solution. All spray bottles were noted to be labeled with 1000 ppm. A few bottles were noted to be at 100 ppm, and one was noted to be at 50 ppm. Please be advised a bleach solution must be made fresh every day and tested using test strips. Please properly label all spray bottles with appropriate concentration.

- 1. Diaper stations were being used for storing boots, creams, phones, and arts and crafts and toys. It was noted that a white diapering pad had turned rainbow due to storage of paint. 2. Multiple rooms indicated that they were washing toys using diapering sinks. 3. Diapering sinks were being used for other purposes besides diapering.Please do not use diapering surface or sink for any other purposes. Please educate staff members on how to properly disinfect diapering area as well sink.

- A designated toy washing sink was not observed in the facility. Multiple rooms indicated that they were washing toys using diapering sinks. The operator also indicated that they use three bins for toy washing. Please designate a plumbed-in sink with hot and cold running water and connected to proper sewer system for toy washing. Toys must be washed using soap and water, rinsed with clean water and then disinfected using appropriate disinfectant. Please do not use diapering sink or bins for toy washing purposes. Alternatively, toys can be washed in the kitchen area once all food activities are completed for the day.

Two aprons, a bath towel and jackets were noted to be stored in the washroom of Explorer 1 and 2. Please remove all items from washrooms.
