- Missing paper towel in the paper towel dispenser at the handwashing station in the waxing room. **Refill paper towel.

- Two nippers in the clean storage unit (UV storage cabinet) was observed to have debris between the nipper heads. **Clean and disinfect thoroughly. Ensure reuseable equipment is scrubbed with soap and water followed by a high level disinfectant. Thoroughly inspect equipment before placing in clean storage unit. ***DIRTY EQUIPMENT WAS REMOVED FROM THE LOCATION AT THE TIME OF THE INSPECTION.

- Staff members noted that they were mixing fabric softener and Preempt RTU concentrate for cleaning of the pedicure basins. **Do not mix fabric softener with Preempt RTU as this was not intended by manufacturer's instruction. Ensure pedicure bowls are cleaned with soap and water following by Preempt RTU disinfectant.

- Client bed in the waxing room was observed to have few plastic packaging left on. **Remove the plastic.

- The reprocessing sink is in disrepair. Wallpaper is used to line the counter of the sink and is lifting from the surface where dirt and debris are lodged underneath and is no longer easily cleanable. Reprocessing sinks must be in good repair for easy cleaning. Repair the sink counter and refrain from covering surfaces with wallpaper or any liner that is not durable, smooth, or easily cleanable.***OUTSTANDING. DUCT TAPE WAS OBSERVED TO BE THE FINISH ON THE COUNTER. THIS IS NOT ACCEPTABLE. ENSURE COUNTER IS SMOOTH, EASILY CLEANABLE AND RESISTANT TO WATER.

- 1. The drill heads used for nails were submerged in isopropyl alcohol. Drill heads should be disinfected with an approved disinfectant such as CS20 in between use to remove contamination. Please ensure the drill heads are disinfected with CS20 for 20 minutes in between clients.2. The operator is using Preempt CS20 disinfectant for reusable equipment and is not submerged in the disinfectant solution for 20 minutes as stated on the manufacturer's instructions. Please ensure staff are following the manufacturer's instructions of the disinfectant to properly disinfect reusable equipment.3. Nail drill handle was visually dirty. The operator stated it is cleaned with water. Using just water is not sufficient to remove contamination for non-critical equipment. Non-critical equipment should be wiped with at least a low-level disinfectant. Following a 2-wipe method is suitable to remove contamination. First wipe is for cleaning the surface to remove debris, and the second wipe is to disinfect with a low-level disinfectant.

- Spray bottles containing isopropyl alcohol solution is not labelled. All chemical solutions must be adequately labeled with the content inside for staff awareness and safe usage of the product. The operator labeled all the spray bottles at the time of the inspection.

Ensure the scoops are adequately cleaned in between use and stored in a manner that protects it from contamination which can further contaminate the bulk supply.

The type of disinfectant being used is considered "ready to use" and does not have instructions for dilution. Prepared high-level disinfectant failed an audit using AHP test strips, suggesting that it had inadequate strength for satisfactory high-level disinfection.Acquire suitable high-level disinfectant. Ensure that all manufacturer instructions regarding its usage are followed. 2) The trays being used to transport clean and used tools were not being disinfected after use.Disinfect storage trays after washing using available low-level disinfectant.
