    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Mackenzie did not notify Alberta Health Services of the existence of her personal service facility.Notification is required for all future personal services operations.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures for personal services workers to follow, including skin cleansing, antiseptic use, and post-service care were not established.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Washroom sink was not supplied with single use towels, or paper towels.Kitchen sink used for hand hygiene was not accessible, and filled with dishes.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Client records for tattooing were not being maintained and kept for at least a period of 2 years.Sterility assurance documentation for pre-sterilized tattoo equipment was not being retained with client records and kept for at least a period of 2 years.

      • Facility was not maintained in a clean and sanitary condition for a personal service facility.- garbage receptables were full- floors were stained with tattoo ink.- cat hair was observed on client contact and work surfaces in the client service area.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • Facility was not maintained in good repair. Client contact surfaces such as the tattoo bed were in disrepair.Washroom hand hygiene sink was not supplied with a sanitary manner to dry hands.Client service area, including storage units containing equipment and sterile supplies were located in the dining area, immediately beside the kitchen where food preparation occurs.
