NO COMPLETE PEST CONTROL LOG BOOK ON PREMISES DURING INSPECTION WITH ALL REQUIRED DOCUMENTATION. ONE PEST INVOICE WAS FAXED TO BUSINESS DATED FROM 09/10. INSTRUCTED MUST HAVE LICENSED PEST CONTROL CO SERVICE ESTABLISHMENT FOR FLIES TO ELIMINATE OR MINIMIZE ACTIVITY AND PROVIDE ALL REQUIRED DOCUMENTATION.

DOOR HANDLE OF ONE DOOR REACH-IN FREEZER BROKEN. MUST REPAIR. MUST NOT USE TINFOIL AS LINER FOR SHELVING IN BASEMENT PREP. GARBAGE CAN CORRALS BEHIND BAR WITH EXCESSIVE DIRT AND FOOD SPLATTER. MUST CLEAN.
