- The dermaplaning blades are past their expiry date (02/2026). Owner states they were just ordered, and they will be contacting the company to rectify. Blades removed from treatment rooms.- Ensure all pre-sterilized equipment is not past the expiry date**Owner contacted supplier and they advised to wipe razor with disinfectant wipe before use as they no longer guarantee sterility of the product. PHI advised this is not acceptable practice of a product intended to be sterile at the point of use.

- Mirrors are present in the laser treatment room posing a potential risk of eye damage due to refraction of the laser.-Ensure all mirrors and reflective surfaces are covered during treatments or removed from the treatment room. A hand mirror may be acceptable if stored face down in a covered location such as a drawer or cupboard.

- Sterility assurance documentation is not being maintained for pre-sterilized piercing equipment- Ensure all services using pre-sterilized equipment have the lot and expiry of the sterile equipment documented with the client's info.

- 1. There are non-critical surfaces being disinfected with isopropyl alcohol. Alcohol is a skin antiseptic and not designed for surface disinfection. Approved low-level disinfectant is available for use.- Ensure an approved surface disinfectant is used on non-critical equipment2. The dremel machines are not being cleaned and disinfected between clients. Visible debris on machine. Single-use disposable pads replaced for each client.- Ensure the dremel machine is cleaned and disinfected with at least a low-level disinfectant between clients.

- A mascara wand was noted soaking in the high- level disinfectant. Staff discarded the mascara wand.- Ensure all single use items are discarded after use on each client.

Alternately, you may have one central reprocessing area for all semi-critical tools to be disinfected using one high-level disinfectant log.
