      • 1. Long, tangling wire was observed in Aspen room.-Please secure or neatly organize all loose wiring and keep long tangling wires out of reach to prevent tripping and entanglement hazards.2. Push pins and staple pins observed in the facility’s corridor.-Please replace the push pins with safer materials for items attached to classroom walls, and cover exposed staple pins with adhesive tape. This will help reduce children’s exposure to potential hazards and prevent injuries.

      • 1. Torn leather sofa chairs were observed in several classrooms.-Please repair or replace the indicated chairs to ensure effective cleaning and sanitizing.2. Debris observed under cubbies and cabinets in Willow classroom.-Please clean the far-reaching areas in this classroom to maintain sanitary conditions.3. A damp area with moisture and a swollen baseboard was observed in the infant washroom.-Please identify and repair the source of the leak, and replace the damaged baseboard to ensure the area is maintained in good repair.

      • Dirty toy containers observed in Willow classroom.-Please clean and sanitize the toy containers in the room.

      • Mouse droppings were observed in the washroom adjacent to the infant room.-Please engage a pest control professional to address and eliminate the pest activity. Ensure that all gaps, holes, or crevices are properly sealed to prevent further entry.-Please provide biweekly or monthly pest control reports, including photos and details of the assessments, to the district inspector.The mouse droppings and clutter observed in the identified washroom were cleaned during the inspection.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Long, tangling wires were observed in the kindergarten room.-Please keep long wires out of children’s reach or secure them properly (e.g., taping them down) to prevent the risk of entanglement.2. Craft bottles with openable lids and others with multiple layers of tape were observed in the baby room. Multiple layers of tape may make the bottles difficult to clean properly.-Please seal the bottles appropriately using suitable glue to prevent children's access to potential hazards.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Surface sanitizer (bleach solution) was measured at 50 ppm in all classrooms.-Please dilute and ensure the bleach solution reaches an adequate concentration of 100 ppm, and conduct daily monitoring using a suitable test strip.Recommendation: A well-labeled, dedicated sanitizer bottle should be provided for the diapering area/surface in both the baby and toddler rooms.2. Some materials were observed on the diapering surface during the inspection.-Please remove all materials from the diapering surface to ensure proper cleaning and sanitizing.

      • Gaps observed in between the floorboards in kindergarten washroom.-Please repair to prevent tripping hazard.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1. Dirty toy containers were observed in baby room.-Please clean and sanitize the indicated containers to ensure sanitary condition.2. Cane rattan toy baskets which could be difficult to properly clean and sanitize, were observed in some classrooms.-Please replace such toy containers with smooth, non-absorbent and easy-to-clean containers.

      • The handwashing sink in the baby washroom was draining slowly.Please repair it to ensure proper handwashing practices.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Children’s bedding/linens were stored in a way that allowed them to touch each other in the toddler room.-Please store them properly to ensure the beddings do not come into contact with each other.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Some children's water bottles were observed without name tags in some classrooms.-Please ensure all children's bottles are labeled with their names.

      • 1. Uneven floorboards were observed in several places in kindergarten washroom: floor wood tiles are lifting up and not properly flushed at the same level.Please fix and ensure good repair.2. The washroom baseboards not properly attached to the wall with exposed nails.-Pease fix to prevent injury and ensure good repair.

      • Art supplies briefly stored on diapering surface. Ensure this area is for diapering only and no other items are stored on it.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • The bin used for sand in the maple room was broken and taped. Ensure this is removed.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1. Hard surface toys in the cherry room are cleaned in soapy water every 1-2 weeks. Cleaning schedules were not maintained. Ensure toys are cleaned at least weekly.2. Water tables are used all day and only cleaned/disinfected once per day. Ensure water tables are cleaned/disinfected twice per day.
