    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • AHS was not notified about the addition of new services.**Ensure to inform AHS prior to the addition of new services.-The operator filled out a personal services notification during the inspection.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Several soiled equipment including combs, brushes, nail buffers were observed in the workstations and in close contact with clean equipment.**Ensure all soiled equipment are removed from the workstations and kept in the reprocessing area for reprocessing or discarded immediately after use as required.**Please have the all the workstations throughout the facility organized and discard/ reprocess all soiled equipment as required.
    • 20. Is reusable equipment disinfected properly?
      • The operator indicated that semi-critical equipment are more often reprocessed using a dry heat sterilizing cabinet.**Ensure all reusable semi critical equipment are disinfected with at minimum a high-level disinfectant at all times. -Please note that the use of dry heat is not an approved method of reprocessing reusable equipment.
    • 20. Is reusable equipment disinfected properly?
      • There were no test strips and no records for testing of the high-level disinfectant available on site.**The high-level disinfectant must be used according to the manufacturer's instructions, and the minimum effective concentration must be tested daily using the required test strips.**Adequate written records for daily testing must be maintained on file at the facility.
    • 20. Is reusable equipment disinfected properly?
      • Reusable non-critical equipment such as comb are not being adequately reprocessed.**Ensure all reusable non-critical equipment are adequately cleaned to remove visible soil and then disinfected using at minimum a low-level disinfectant.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The facility was not maintained in a visibly clean and organized manner.**Please have all the workstations and storage areas cleaned and organized.**Please declutter the facility and remove/ put away all equipment not being used for the provision of services.
