      • Cosmetic inks for PMU artist are not date marked after opening. One color pigment (Adora) bottle did not have an expiry date. **Lash services are suspended by operator at this time until a new lash artist is acquired. At that time, please contact AHS to review setup and procedures

      • Used emory boards were stored in drawers at nail stations.*Ensure all single-use equipment is discarded after use on client.

      • Several soiled equipment including emory boards and makeup brushes were observed in the workstations and in close contact with clean equipment.**Ensure all soiled equipment are removed from the workstations and kept in the reprocessing area for reprocessing or discarded immediately after use as required.

      • Facility a bottle of 50% isopropyl alcohol (no NPN/DIN) Discontinue use of 50% isopropyl alcohol- this product does not contain a NPN or DIN.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Cosmetic inks for PMU artist are not date marked after opening. One color pigment (Adora) bottle did not have an expiry date. *Ensure inks are labelled with expiry date from manufacturer and, once opened, label with 'date opened' to follow use-by date marking by manufacturer
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The facility was not maintained in a visibly clean and organized manner. Stains on work stations, chairs and carts. Work stations had used equipment that was not removed after the service was completed. **Please have all the workstations and storage areas cleaned and organized.**Please declutter the facility and remove/ put away all equipment not being used for the provision of services.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • Paper towel was not stored at handwash sink in the studio room. Corrected onsite. Ensure paper towels are stored in a dispenser for staff use when washing hands in the designated handsink.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Unlabeled bottle by nail station. Ensure all bottles are labeled to identify its content/use.

      • AHS was not notified about the addition of new services.**Ensure to inform AHS prior to the addition of new services.-The operator filled out a personal services notification during the inspection.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Several soiled equipment including combs, brushes, nail buffers were observed in the workstations and in close contact with clean equipment.**Ensure all soiled equipment are removed from the workstations and kept in the reprocessing area for reprocessing or discarded immediately after use as required.**Please have the all the workstations throughout the facility organized and discard/ reprocess all soiled equipment as required.
    • 20. Is reusable equipment disinfected properly?
      • The operator indicated that semi-critical equipment are more often reprocessed using a dry heat sterilizing cabinet.**Ensure all reusable semi critical equipment are disinfected with at minimum a high-level disinfectant at all times. -Please note that the use of dry heat is not an approved method of reprocessing reusable equipment.
    • 20. Is reusable equipment disinfected properly?
      • There were no test strips and no records for testing of the high-level disinfectant available on site.**The high-level disinfectant must be used according to the manufacturer's instructions, and the minimum effective concentration must be tested daily using the required test strips.**Adequate written records for daily testing must be maintained on file at the facility.
    • 20. Is reusable equipment disinfected properly?
      • Reusable non-critical equipment such as comb are not being adequately reprocessed.**Ensure all reusable non-critical equipment are adequately cleaned to remove visible soil and then disinfected using at minimum a low-level disinfectant.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The facility was not maintained in a visibly clean and organized manner.**Please have all the workstations and storage areas cleaned and organized.**Please declutter the facility and remove/ put away all equipment not being used for the provision of services.
