    • R10.4 - Are restraint and fall prevention plans appropriate?
      • Observation(s): Fall care plan interventions were noted to be in place for persons in care assessed as high risk for falls, or with a history of falls. Persons in care noted to require Universal Fall Precautions were noted to have the interventions in place, however, the interventions were not noted in their care plans. Lack of detailed fall interventions in a care plan could lead to increase risk of falls for persons in care. Submit by May 1, 2026 a written plan outlining how you will ensure all persons in care have fall interventions outlined in their care plans. The plan must include the system you will implement to support ongoing monitoring to ensure sustained compliance.

      • Observation(s): Oral care plans were noted to provide minimal direction. They did not outline what sort of care or level of support was required. Without an informative oral care plan, persons in care may not receive the level of support or assistance required to maintain oral health. Submit by May 1, 2026, a written plan outlining how you will ensure all persons in care have an oral health care plan with clear directions for staff. The plan must include the system you will implement to support ongoing monitoring to ensure sustained compliance.
