    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Ensure written procedures are filled out and readily available upon request for review.- Will provide operator with written procedure template.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • 1. Did not have cleanser onsite for skin cleansing.- Operator left and acquired skin cleanser.2. One bottle of alcohol antiseptic did not have NPN or DIN and several smaller bottles were expired March 2025.- Operator was advised to not use alcohol antiseptic without any NPN or DIN and ensure to keep track of expiry dates.- Operator left and acquired alcohol with NPN and not expired. Remove expired and non-approved antiseptic from facility.
    • 20. Is reusable equipment disinfected properly?
      • No test strips or recordkeeping of CS20 disinfectant.- Acquire test strips and ensure testing is recorded on record log. Will provide record log to operator.
    • 20. Is reusable equipment disinfected properly?
      • 1. Use of Mr. Clean for soaking reusable instruments as disinfectant.- Refrain from using Mr. Clean as a disinfectant for reusable instruments. Use the high level disinfectant onsite (CS20) and ensure a 20 minute contact time is achieved.- Operator was instructed to remove the Mr. Clean, rinse and clean the soaking container and use CS20 for disinfecting. All instruments that regularly go through reprocessing was rinsed, cleaned, and disinfectant with CS20.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • Update Aug 22, 2025:- Still outstanding. Please consult with manufacturer for label and manufacturers instructions.The microdermabrasion machine has no manufacturer's product name label on it and no manufacturer's instruction or operative manual available onsite.-Please ensure the microdermabrasion machine has the manufacturer's label on it and have its operating manual onsite as well.The operating manual of Rido Ozone Facial steamer was not available onsite.-Please have available onsite the steamer operating manual.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • Sink faucet in first waxing room was not secured to sink basin.- Fix the sink faucet and ensure it is in good repair.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Spray bottle of floor cleaner was stored on top of service cart in first waxing room.- Find a separate storage area for floor cleaner not on or near service cart to prevent contamination.
