      • In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. OBSERVED SEWAGE WATER ON BASEMENT FLOORS IN REAR AREA. MUST CONTACT LICENSE PLUMBER, REPAIR,CLEAN/SANITIZE FLOORS THRU-OUT AREA.
    • 11. ADEQUATE NUMBER, CONVENIENT, ACCESSIBLE, DESIGNED, AND MAINTAINED
      • Adequate and convenient toilet facilities shall be provided. They should be properly designed, maintained, and accessible to employees at all times. MUST INSTALL HAND SINK BEHIND BAR WITH COLD /HOT RUNNING WATER UNDER CITY PRESSURE./SOAP AND HAND DRYING DEVICE.

      • All food should be properly protected from contamination during storage, preparation, display, service, and transportation. OBSERVED ICE NOT PROTECTED DURING STORAGE,BEER STORED INSIDE ICE MACHINE. MUST REMOVE ICE.
    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. MUST RODENT PROOF FRONT DOOR OF PREMISES. ALSO MUST PROVIDE PEST CONTROL LOG BOOK FROM PEST CO.

      • The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OBSERVED OUTSIDE AREA /YARD NOT MAINTAINED,EXCESSIVE HARBORAGE-ARTICLES. MUST REMOVE,CLEAN TO PREVENT PEST HARBORAGE AREA.

      • When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code. NO HOT WATER AT HAND SINK IN MEN"S WASHROOM. MUST REPAIR.

      • All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST REPLACE BOTTOM INSIDE CABINET IN WOMENS WASHROOM.

      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN DEBRIS FROM FLOORS UNDER BAR,ALL STORAGE AREAS. MUST SEAL AROUND PIPES UNDER BAR.

      • The walls and ceilings shall be in good repair and easily cleaned. MUST REPAIR LOWER WALL UNDER UNRAL IN MEN"S WASHROOM,AND SEAL GAPS ALONG BASEBOARD IN MEN"S WASHROOM.
    • 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
      • The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST INSTALL MOP SINK,COLD/HOT RUNNING WATER UNDER CITY PRESSURE. MUST INSTALL VENTILATION IN LADIES WASHROOM.

OBSERVED EXCESSIVE CLUTTER IN REAR STORAGE AREA,POTENTIAL PEST HARBORAGE AREA. MUST REMOVE ANY UNNECESSARY ATTICLES,EQUIPMENT FROM AREA. MUST ORGANIZE/ELEVATE REMAINING STORAGE.
