- Observation(s): At times, persons in care due to personal choice are not eating the breakfast or lunch noted in the posted menu, and the alternate food offered is not always being documented. If substitutions to the posted menu are not documented, it can be difficult to properly monitor the nutrition of persons in care. Submit a corrective action plan by March 20, 2026, which indicates what systems will be put in pace to ensure food substitutions are documented.

- Observation(s): The picnic table on the outdoor deck was observed as having chipped paint and a rough surface. The picnic table surface poses a potential risk for slivers to persons in care; it would also be difficult to properly sanitize/disinfect. Submit a corrective action plan by March 20, 2026, which indicates what steps will be taken to ensure all outdoor equipment is kept in a good state of repair.

- Observation(s): In a review of facility Medication Administration Records for the the month of January 2024, one instance of the charting of the result of a PRN medication was noted as incomplete. During the inspection follow-up was completed by the facility manager to gain the missing information. No follow-up required as the issue was corrected during inspection.

- Observation(s): During review of Medication Safety and Advisory Committee (MSAC) meeting minutes, the last recorded meeting and medication room/cupboard audit took place in April 2021. Please ensure an MSAC meeting is scheduled, and meetings occur on an annual basis.

- Observation(s): Some PIC's have not been weighed recently as the facility does not have access to a scale specific to their needs. Prior to COVID the facility was able to utilize another care facilities scale, since that time the weights have not been taken. Please develop a plan to ensure PIC's are weighed monthly and the weights are updated in PIC nutritional care plans.

- 4.2 Are Medication Administration Records accurate, kept on site and are adverse events documented and reported?
- Observation(s): The MAR record for one PIC reviewed by Licensing was incorrect and did not show the time of administration of medication for one item for the previous day, as it is the 2nd day of the month a new MAR had just been received, the medication was given and not charted, action was taken at the time of this inspection to have a new MAR sent from the Pharmacist. Ensure staff follow facility procedures when a MAR is found incorrect to inform the Pharmacy immediately to have the mistake rectified.
