- Observation (CORRECTED DURING INSPECTION): Noted a dish rack that was coming apart with plastic pieces hanging off the rack.
- Corrective Action(s): Operator discarded the rack during the inspection.
- Reminder to check all equipment to ensure they are in good condition so no pieces can come off and become a physical contaminant to food.

- - Black buildup noted along the edge of the sinks beside the dishwasher. - Debris and buildup noted in most of the handwash stations (faucets and soap dispensers) - notably in stations 7, 8 and 11.
- - Debris noted on the wall under the prep table - across from the dishwasher.
- Unit 1 -
- - Debris noted on the floor of the common areas.
- - Around the caulking of the 2-compartment sink beside the dishwasher. - Debris and buildup noted in the unused reach-in cooler in the front station - currently not occupied.
- Corrective Action(s): Clean and sanitize the abovementioned areas and maintain in a sanitary condition.

- Observation: No Sink & Surface Cleaner Sanitizer detected in one of the wall mounted dispenser (unit closest to Namak Daani).
- Corrective Action(s): Fix the dispenser so that the appropriate concentration is dispensed when used. Note: The second wall mounted dispenser by the dishwashing area was functioning and noted with appropriate concentration. Bleach was also available to be used as a sanitizer.

- Observation: One of the wall mounted dispensers for Sink & Surface Cleaner Sanitizer (by dishwashing area) was noted with a concentration higher than permitted as per the manufacturer's label.
- Corrective Action(s): Operator must adjust or have the dispenser service so the correct concentration can be attain at this unit.

- 4. Frost buildup noted in the interior of the 3-door freezer in the front unit (by the washrooms).

- Corrective Action(s): Clean and sanitize the abovementioned areas and maintain the cleanliness.
