      • All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE BAR SODA GUN HOLDERS ARE DIRTY, WASH, RINSE AND SANITIZE THE SODA GUNS AND HOLDERS.

THE KITCHEN FLOORS ARE DAMAGED BY THE HOT LINE AND DIRTY BEHIND THE EQUIPMENT LEGS. THE REAR DOOR WALKWAY IS GREASY. RESEAL THE KITCHEN AND DOORWAY FLOORS SO THEY ARE SMOOTH, SANITARY AND EASY TO MAINTAIN.

      • The walls and ceilings shall be in good repair and easily cleaned. THE WALLS ABOVE THE CATCH BASIN HAVE OLD SMALLFIY RESIDUE, CLEAN AND REPAINT THE WALLS.

      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Over 40 live smallflies were found in the basement ejector pit and 10 were found in the bar, Anderson Pest Control report dated 8/5/10 also documented smallfly activity.
