      • Single use tools such as mascara wands are being reprocessed and reused.**Ensure these items are discarded after each use.

      • 1. Clean/reprocessed equipment such as combs, brushes, scissors and other hair equipment were being stored in drawers which had an accumulation of hair.2. Reprocessed semi-critical equipment such as tweezers and extractors were being stored in a soiled container.**Ensure clean/reprocessed equipment are stored in a location/manner that protects them from contamination.

      • The cloth bed liners are not being changed after each client.**Ensure the cloth liners are changed after each client. Alternatively, revert to using just the bed surface and paper liners as the bed surface itself is made of cleanable material. Ensure to disinfect the bed surface after each client.
    • 20. Is reusable equipment disinfected properly?
      • The high-level disinfectant is not being used according to the manufacturer's instructions. The CS20 decanted has been used for 16 days.**Ensure manufacturer's instructions are followed and have the CS20 discarded 14days after pouring it.
    • 20. Is reusable equipment disinfected properly?
      • The high-level disinfectant was not completely covering the reusable semi-critical tools being reprocessed.**Ensure the tools are fully immersed in the disinfectant during reprocessing.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Several drawers in the hair dressing area had an accumulation of hair of hair inside them.**Please have these areas/items cleaned.

      • The high-level disinfectant (CS20) is not being tested daily. Records on site indicate the decanted disinfected is tested once every 14 days.**Ensure the high-level disinfectant is tested and recorded daily. ** In line with the manufacturer's instructions, the disinfectant is to be discarded 14days after pouring, when visibly soiled or after a failed test.
    • 20. Is reusable equipment disinfected properly?
      • Disinfectants are not being used according to the manufacturer's instructions. Clippercide is being wiped off immediately after application, not allowing for the required contact time.**Ensure all disinfectants are used according to the manufacturer's instructions.**Ensure to clean and remove all visible soil from your equipment before applying the clippercide disinfectant. The equipment must be kept wet with the disinfectant for a minimum of 10 minutes before wiping off.

      • 1) A user manual was not readily available for the Class 4 Laser machine.REQUIREMENT: Obtain a user manual for the machine.2) There was no signage for the door to indicate laser services would be ongoing, and there was a large mirror located inside the laser room.REQUIREMENT: Obtain proper signage, remove the large mirror from the room, and ensure all manufacturer's instructions for safety and use are followed.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • A cleaning solution to run through the lines of the hydrofacial machine was not available.REQUIREMENT: Obtain a cleaning solution that has either a sanitizer or disinfectant in the solution which will ensure that the lines of the machine remain sanitary.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • A notable build-up of scale was noted in a facial steamer. The operator was not using distilled water in the machine.REQUIREMENT: Make a water-vinegar solution and run it through the machine to clean it. After treatments are complete, ensure the continued use of distilled water for the facial steamer.
