      • CS20 was expired May 28, 2025. Instructed staff to acquire new CS20 for the same day and to inform staff to not use expired CS20 for disinfecting.- Operator was able to provide photo of new disinfectant bottle with non-expired label after the inspection was conducted.

      • 1. No test strips available for high level disinfectant CS20.- Acquire test strips for daily testing of disinfectant.2. No recordkeeping log of testing of high level disinfectant. - Ensure a daily record log after each test.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Waxing room is cluttered and needs cleaning and reorganizing.- Clean and reorganize the waxing room.
