      • A used nail buffer was found on the storage cart.The used nail buffer was discarded. Do not reuse single use items.
    • 20. Is reusable equipment disinfected properly?
      • Stainless steel foot files were disinfected in a barbicide jar; however, the jar was too small to accommodate the foot files.Obtain a larger container so that reusable instruments can be submerged in the disinfectant.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Pens were stored in the same container as single use lash brushes.Pens were placed separately during the inspection.

    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • The hand sink in the wax room had no paper towels. Paper towels were provided during the inspection. Provide a suitable dispenser for storing paper towels.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • The operator stated that cleansing of the skin was not performed on clients prior to offering wax services. Cleanse the client's bodily area before waxing.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • 1) A used, single use nail file was found in a drawer of a nail station.2) Used, single use applicators for lashes were tucked into the headrest pillow.3) Used, lash glue cup holders were attached to a cart. Used, single use items were discarded during the inspection.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • 1) New, single use applicators in storage were contaminated with hair and lash extensions. 2) There was hair on wax strips and single use wooden applicators that were stored on a shelf.Contaminated, single use items were discarded during the inspection. Protect single use or reprocessed equipment during storage.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • A reusable foot file that had been cleaned and disinfected had a gel-like substance on hard-to-reach areas.The foot file was disassembled to its simplest form for cleaning and disinfection during the inspection.
    • 20. Is reusable equipment disinfected properly?
      • 1) CS20 disinfectant expired on May 2, 2025.The disinfectant was discarded. The facility had another high-level disinfectant that was not expired.2) There were no test strips for monitoring the concentration of high-level disinfectant during the reuse period.Provide test strips and monitor the disinfectant concentration daily.3) A log for recording the concentration of high-level disinfectant during the reuse period was not available.Maintain written record daily.4) There was floating debris in the disinfectant tray.Disinfectant was replaced during the inspection. Ensure to clean reusable instruments properly prior to disinfection.5) Reusable makeup mixing palettes and brow scissors were being disinfected with 70% isopropyl alcohol. After cleaning, disinfect with a low-level disinfectant at minimum. Ensure that the label of the low-level disinfectant has a DIN and the word "disinfectant".6) The operator was observed using a CS20 disinfectant for the foot basin without following the minimum contact time. The disinfectant was sprayed then wiped off immediately.The foot basin was disinfected with a low-level disinfectant while following manufacturer's instructions for use.

      • A tube of lash/ brow tint was stored without a cap. Product had hardened.Product was discarded.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The floor of the wax room was sticky.Clean the floor more frequently.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Ready to use disinfectant and soapy water in spray bottles were not labeled to indicate their contents.Label all chemical bottles.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • In the lash room, the headrest pillow was in a state of disrepair and there was a dirty cushion on the technician's chair.Properly repair or replace the headrest pillow.Clean the cushion. Cushion or pillow protectors can be used.
