      • Facility was operating for the last 5 years without notifying the health department.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility information on the microneedling cartridge was not retained on the client's file for tractility.- Ensure sterility information, including lot number and expiration date, are retained.

      • Cosmetic Botox by Allergan was reconstituted with saline with a preservative, the product would be kept in refrigeration for 2 weeks.- Botox was not following manufacturer instructions of reconstitution with preservative-free saline and discard after 24 hours.Operator will follow manufacturer instructions of reconstitution as May 07, 2026.
