- A high-level disinfectant (HLD) is not available and is required for disinfection of semi-critical equipment (i.e. eyelash extension tweezers). Purchase a high-level disinfectant (HLD). Most HLD’s have a 14 to 30-day reuse period but you must also retain proper indicator test strips and keep HLD logs. If using PreEmpt CS20, buy AHP7 indicator test strips (50 count). If using PreEmpt HLD5, buy AHP2 indicator test strips (100 count). HLD log templates will be provided. Semi-critical equipment is not permitted to be re-used until a proper HLD has been obtained.

- Cosmetic products used for eyelash / eyebrow tinting services are not permitted for use of dyeing lashes/brows as per manufacturer's instructions. Operator agreed to remove products from the facility at the time of inspection.

- 1. Cosmetic products at the pedicure are uncovered and have sticks within the jars that could contaminate the bulk supply. Remove sticks and ensure covers are obtained.2. The bottle at the pedicure station allegedly contains pedicure salts. Label this decanted container. Additional labelling information required - bulk supply is not at the facility. Ensure decanted cosmetic products are labelled with its contents. The original/bulk supply must be kept on-site and labelled with product identity, manufacturer, directions for use and list of ingredients.3. Additional cosmetic products are required (i.e. Oxidant 3% developer) for use with refectocil tint according to label instructions. Obtain proper cosmetic products and follow label instructions.4. Henna tints used for eyebrow henna services are missing labelling information including instructions for use. Obtain the following labelling requirements: product identity, manufacturer, directions for use and list of ingredients.5. Some of the waxing pots in various areas throughout the facility were missing lids which could contaminate the bulk supply. Obtain lids.
