- 1. In various areas throughout the salon, numerous unlabeled spray bottles contained decanted disinfectant. Please label decanted bottles of disinfectant with the contents, DIN and expiry date. The original/bulk supply must be kept on-site to verify labelling requirements.2. Room #3 contained antiseptic (Medi Care 70% rubbing alcohol NPN: 80021572) with a worn label - the expiry date was not legible. Requested removal from the facility.

- 20. Is reusable equipment disinfected properly?
- 1. There were 3 high-level disinfectant trays in the manicure/pedicure area. High-level disinfectant logs were not being maintained for each tray used. Consolidate the high-level disinfectant trays and/or ensure high-level disinfectant logs are being maintained for each tray.2. Proper indicator test strips were not available. Obtain AHP7 indicator test strips (50-count) if using PreEmpt CS203. The high-level disinfectant (HLD) tray was too small for the large “cheese-grater” style metal foot files. Equipment must be fully immersed. Obtain a larger disinfectant tray (plastic/glass with a lid).4. Upon removing the removable magnetic fan within one of the pedicure foot basins, there was visible build-up of debris. Please clean and disinfect the foot basin. Ensure foot basins are fully disassembled into its components for proper. If the removable magnetic fans are no longer used, remove these components entirely from the facility.

- The manufacturer’s instructions for use (MIFU) was not available for the O'melon Omega light in room #2. Obtain the MIFU and have it available for the next scheduled inspection.The MIFU for all energy-emitting equipment must be kept in an accessible place for reference by a personal service worker at all times.

- Tints not suitable for eyelash and/or eyebrow tinting were discarded at the time of inspection.

- 1. The waxing pots in Room #1 were uncovered. Requested a cover to prevent contamination of the bulk supply.2. Decanted cosmetic products in various areas throughout the salon were not labelled with its contents. Labelling requested. Ensure decanted cosmetic products are labelled with its contents. The original/bulk supply must be kept on-site to verify labelling requirements.3. The LaPalm 70% isopropyl alcohol spa product in the manicure/pedicure area had a worn expiry date that was no longer legible. This product was also being decanted for us as an antiseptic for esthetic services. According to label instructions, warning states "do not get on skin".4. Dirty spoons/sticks were being left in cosmetic products which may contaminate the bulk supply. Discard spoons/sticks after each use.5. Elleebana cosmetic products used for lash lift/perm cannot be used for eyebrow lamination services unless stipulated on the label. Obtain suitable cosmetic products for eyebrow lamination services.
