      • The facility staff reported the pest control operator identified the flies present as drain flies.Recommended:-the grease trap access lid seal was in poor condition and a vent stuck through. Ensure the lid completely prevents flies from coming and going-the soil around the grease trap appeared moist. Ensure the tank is not leaking and that the grease trap cleaning person spills as little as possible-the grease trap round lid did not appear fully seated. Ensure it is closed tightly-continue to consult with the pest control operator to eliminate flies

      • -facility was using iodine in the bar glass washer. The iodine bottle was empty and was replaced. The facility did not have test strips for the glass washer and was not testing the iodine concentration. The glass washer was tested using the inspector's test strips and the concentration measured 0ppm. Please correct all of these items

      • -floors were dirty-container lids and sides were dirty

The last recorded pest control service on site was on June 23, 2025.– Schedule the monthly routine service immediately to address the current pest activity.
