- 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
- Facility had changed ownership and had not submitted a personal services notification. Inspector submitted a personal service notification on behalf of operator following inspection.
- 20. Is reusable equipment disinfected properly?
- Silicone pads used for lash lifts were being disinfected with an intermediate level disinfectant. These must be disinfected with a high level disinfectant. Ensure they are rinsed with water after disinfection. Replace when worn/cracked.
- 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
- Mouse droppings were observed in back area around electrical panel and in back storage room. Clean all droppings and monitor for any new pest activity. Facility had previously contracted a pest control professional. If new activity is noted, ensure pest control company is contracted. Seal any potential entry points including under exterior door and any gaps around electrical panel.
