    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • It was also noted that the facility has new additional services, and an updated Personal Services Notification (PSN) was not received.Please submit a PSN.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • 1. Written cleaning and disinfection procedures were not observed. 2. Written skin cleansing and antiseptic procedures were not observed. 3. Written blood and body fluid procedures were not observed.Please create written procedures, submit them to this office and implement them. Please ensure they are easily accessible.
    • 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
      • It was noted that the facility was not following MIFU for cosmetic products. Alternative written procedures were not established.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • 1. Antiseptic: Hydrogen Peroxide (NPN 02004178), and Stanhexidine (02246950) were noted onsite. A few of these products were noted to be expired. Please remove all expired products and follow MIFU. 2. Written skin cleansing and antiseptic procedures were not observed in the facility. Please create written skin cleansing and antiseptic procedures.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance records for BD needles were not observed.Please ensure to record sterility assurance records of all critical disposable pre-sterilized equipment. Please ensure not to open additional boxes with separate lot numbers and stock reusable containers until all stock is used up.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Reusable equipment (ultrasonic gel reusable containers and other reusable storage containers) was not being cleaned and disinfected.Please be advised all reusable equipment must be cleaned with a soap and water and rinsed with clean water. All equipment then must be disinfected using appropriate disinfectant. Please create written cleaning and disinfection procedures for all reusable equipment.
    • 20. Is reusable equipment disinfected properly?
      • 1. Disinfectant products such as caviwipes, Oxivir spray were noted to be expired. These were discarded during the inspection. The facility has extra supply of Caviwipes XL with three minutes contact time and all client rooms were equipped with these during the inspection. Please ensure to follow manufacturer's instructed contact time.2. Reusable equipment was not being cleaned and disinfected. Please be advised that all reusable equipment must be cleaned using soap and water, rinse with clean water and then disinfected using an appropriate disinfectant. Please ensure to maintain manufacturer's instructed contact time of disinfectant product.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. A lot of cosmetic products were noted to be expired. Please remove all expired cosmetic products.2. MIFU of most products were not observed.Please ensure that all cosmetic products MIFU are present and easily accessible.3. Limelight CB-EVS IEUL (Bio Sciences Inc.). ingredient list was not present on the bottle and MIFU were not available onsite. Please ensure that MIFU are available and are being implemented. Please ensure that all cosmetic products have ingredient list. 4. Exohealer Fillcore - according to MIFU, this product requires refrigeration. The product was not stored in the refrigerator and MIFU were not being followed.Please follow MIFU.5. Heaiqui - a freeze-dried powder, ingredient list and MIFU were not observed. The bottle has Korean language. Please discard this product.6. Saline that is being used for neuromodulators (Nuceiva and Dysport) was noted to have preservatives. The facility was not following MIFU which indicates to use preservative free saline solution. This saline did not have MIFU and was noted to be open. The date of opening the vial was not observed. Please follow MIFU. Please ensure that saline solution that do not have use within days after opening must be used within 28 days.7. Multiple Slaine solutions were noted to be expired. Please discard all expired products and follow MIFU. 8. A few Antiseptic (Lidocaine) tubes were noted to be expired. Please discard all expired products and follow MIFU. 9. Alcaine eyedrops were noted to be expired. Manufacturer's instructions for use (MIFU) were not being followed.Please discard all expired products and follow MIFU.10. Lidocaine (DIN 92449684) was noted to be expired. Please remove all expired products and follow MIFU.11. Multiple preservative free Salines were noted to be expired. Please remove all expired products and follow MIFU.
