      • The following areas were in disrepair:- Ceiling in the back dry storage was water damaged and possibly mouldy inside - must replace all water damaged ceiling tiles; if there is any water leak above must be repaired first.- walls in men's public washroom next to the bar had lots of holes in it - must be repaired

      • The following areas were in disrepair:- hand sink cupboard was water damaged, rotten at the bottom with water leaking inside the sink cupboard - water leak must be repaired and sink cupboard must be replaced.- Ceiling in the back dry storage was water damaged and possibly mouldy inside - must replace all water damaged ceiling tiles; if there is any water leak above must be repaired first.- walls in men's public washroom next to the bar had lots of holes in it - must be repaired

      • Pest control checklist filled out and pest activity was recorded for the last two months but no typeof pest/corrective action was documented.Please ensure pest control checklist is completed properly.Room next to the walk-in cooler - room was disorganized and a gap was observed on the doors leading to the outside - the room must be cleaned up/organized and the door sealed to prevent entry of pest.

      • Pizza prep cooler was at 16C - temperature measured with probe thermometer.Small amount of cold cuts was discarded - other foods were still below 4C and manager was instructed to transfer it to another fridge.Pizza prep cooler must not be used until capable of maintaining temperature 0-4C.

      • Cleaning glasses - there were some glasses in the clean glasses storage space that were not properly cleaned.Please ensure soaking/prewash step is properly done before putting the glasses in the dishwasher.

      • Some mice activity observed inside walk-in cooler. A small bag of shredded cheese discarded due to possible contamination (mouse chew on the bag).Pest control checklist not filled out for November. Also, when pest activity is discovered, corrective action must be recorded on the pest control checklist.

      • Few mouse droppings observed in the dry storage areas and inside walk-in cooler.A dead mouse observed in a mouse trap.Trap with a dead mouse removed from the kitchen during the inspection.Please ensure all mouse droppings are cleaned and keep monitoring daily for any new pest activity.Pest control checklist was not filled out for the month of January 2024 - pest control record must be maintained as required.

      • There was no ready to use bleach solution available in the kitchen - fresh solution prepared during the inspection. Please ensure that ready to use bleach solution is prepared at the opening time.Paper towel is used for wiping the food contact surfaces.Bucket with reusable wiping cloths had no detectable sanitizer - corrected immediately.

      • Commercial dishwasher was not dispensing chlorine sanitizer - the machine must not be used until it works properly; Alternative manual 3-compartment sink dishwashing must be followed until the dishwashing machine is repaired.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Significant fly infestation was observed throughout the facility; back door in the bar area left slightly open.A mouse was observed running inside the hole in the floor by the back door and some mouse droppings were noted in the dry storage. Operator was instructed to clean the droppings and sanitize the surfaces immediately and to block off the hole in the floor and set up mouse traps which was done in an hour.Written pest control program was not available for review. Must be established and records available for review during routine inspections.The doors must be kept closed/pest proof to prevent entry of pests.

      • There was some mold observed inside the walk-in cooler on surfaces and alcoholic beverages.Written sanitation procedures/cleaning frequency checklist were not available.Walk-in cooler must be cleaned as required and written cleaning program/checklist started and used.
