- The following deficiencies were noted:1. The high‑level disinfectant (CS20) expired in October 2025.2. There was no visible expiration date on the Preempt RTU bottle.3. Unable to verify whether the high‑level disinfectant is being tested on a regular basis or discarded after each use, given the presence of an almost full bottle of expired CS20 solution.Please obtain new CS20 and Preempt RTU disinfectants. Additionally, ensure that the CS20 disinfectant is tested daily using the appropriate test strips or discarded after each use. If the CS20 solution is being re‑used according to the manufacturer’s instructions for use, a daily log must be maintained and completed each day.

- 1. Preempt RTU was being used to disinfect reusable semi-critical equipment such as nail cutters and cuticle clippers.-Please use high-level disinfectant such as preempt CS20 to clean such tools. Preempt CS20 was available onsite during the inspection.2. A lot of dirt residues/sediments was observed in the dispensed solution used to disinfect reusable semi-critical equipment. Used equipment were not cleaned prior to disinfection.-Please thoroughly clean all equipment prior to disinfection step, ensure to follow manufacturer's instructions for usage.3. The high-level disinfectant's test strip availble onsite had expired.-Please provide and use effective test strip to ensure the efficacy of high-level disinfectant when being reused.

- Preempt CS20 was used to disinfect facility's surfaces and pedicure basins.-Preempt CS20 is a high-level disinfectant and is meant to disinfect reusable semi-critical tools such as nail clippers or foot files. Please procure and use disinfectant meant for hard surfaces such as preempt RTU.
