      • Violation 115 corrected, mouse droppings were cleaned and sanitized and the syrup leak was cleaned up adequately.
      • Ensure to submit the written pest control plan with the mapped out spots for the current traps inside the building no later then may 29th 2026. No other issues were noted. Comments

      • Pest droppings noted inside the wooden boxes that contained syrup in carboard box and the syrup was leaking.
      • 1. Disconnect and discard the affected syrup boxes immediately.(CDI) 2. Ensure to clean up and sanitize the areas that were affected by the feces by using bleach solution diluted to 1:10 .

      • Pest droppings were noted on the floor by the hot water heater side of wall and across where the freezers are located.
      • Ensure to clean and sanitize the area where the droppings are noted. Comments Violation 116 corrected, door sweeps were installed, and the gaps in the ceiling of the basement were sealed. No concerns in the kitchen were noted. As discussed, create a pest control plan before May 29th, 2026 and, more information to follow on the email.
