      • October 14, 2025: An accumulation of nail dust was observed on surfaces in the manicure stations, a build-up lotion observed on the handle of the towel warmer, dust observed in the washroom area. - Clean and maintain in a clean and sanitary condition at all times.- All client contact surfaces must be disinfected after each client using a low-level disinfected such as Lysol wipes. July 10, 2025: Some cleaning had been done. More required. July 9, 2025: Unsanitary conditions found throughout the facility including but not limited to: an accumulation of nail dust on surfaces in the manicure stations, dirty paraffin and cleanser containers, dirty floors and walls. - Clean and maintain in a clean and sanitary condition at all times.

      • July 10, 2025: Some cleaning had been done. More required. July 9, 2025: Unsanitary conditions found throughout the facility including but not limited to: an accumulation of nail dust on surfaces in the manicure stations, dirty paraffin and cleanser containers, dirty floors and walls. - Clean and maintain in a clean and sanitary condition at all times.

    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • Written protocols for disinfection on the wall in the back room near the sink were blank and not being followed. Complete a step-by-step instruction for cleaning and disinfecting tools and pedicure tub and train all personal service workers and provide confirmation of training.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Used nail files, sanding bands are stored in the manicure table drawers and storage. Ensure single-use items such as nail files, sanding bands that cannot be cleaned properly are discarded after use.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Inadequate cleaning of tools and equipment prior to submerging into disinfectant evident by presence of several nail in the disinfectant solution. - All reusable equipment and tools must be washed with detergent and rinse with water. Next dry the tools prior to soaking into the disinfectant solution.
    • 20. Is reusable equipment disinfected properly?
      • 1) The concentration of CS20 in disinfection container was measured at 0%. 2) Testing records required to assess the efficacy of the high-level disinfectant Preempt CS20, were not maintained. The CS20 log was blank. 3) The CS20 concentration is not being tested daily. - The solution must be discarded daily or discarded after 14 days or sooner as dictated by AHP7 Test Strips.3) An intermediate disinfectant was not available for disinfection of pedicure basins. - Ensure pedicure tubs are cleaned with soap and water, dried, sprayed with an intermediate-level disinfectant such as RTU for an adequate amount of time as required by manufacturer, rinsed, and dried.4) Contact time required for Preempt CS20, is not being achieved. Operator indicated tools are submerged in the solution for up to 15 minutes before removing.- Follow the manufacturer's instructions. A 20-minute contact time is required.

      • Facility has some written protocols for client care on the wall in the back room near the sink. However, they are water damaged and not being accurately followed. Please complete a step-by-step instruction for cleaning and disinfecting tools and pedicure tub and train all personal service workers on them.

      • Used nail files, sanding bands are stored in the manicure table drawers and storage. Ensure single-use items such as nail files, sanding bands that cannot be cleaned properly are discarded after use.
    • 20. Is reusable equipment disinfected properly?
      • 1) Pedicure and manicure re-usable equipment are not disinfected adequately. CS20 and RTU disinfectants that were in use were both expired.- Operators were able obtain disinfectant while PHI was on-site. - A high-level disinfectant such as PREempt CS20 is required for semi-critical tools such as pedicure and manicure instruments. Follow the manufacturer's instructions. Equipment must be cleaned with detergent and hot water, then rinsed with hot water, dried, prior to submersion in the CS20 for minimum of 20 minutes. CS20 solution must decarded daily.2) Pedicure tubs are washed and rinsed only and not disinfected. RTU was expired. - Operators were able obtain disinfectant while PHI was on-site.An intermediate-level disinfectant such as PREempt RTU. Follow the manufacturer's instructions. Tubs must be cleaned with detergent and hot water, then rinsed with hot water, dried, prior to applying the RTU for 3 minutes.
