      • The high-level disinfectant - Preempt HLD5 was expired. The high-level disinfectant must not be expired to ensure adequate disinfection of semi-critical tools. Please obtain.

      • 1. Observed the operator placing the soiled semi-critical tool (cuticle clippers) directly in the high-level disinfectant without cleaning first. Informed the operator to remove the cuticle clippers and clean the tool first before placing inside the high-level disinfectant. 2. The operator then rinsed the cuticle clippers with only water, then placing it directly in the high-level disinfectant without drying. According to the manufacturer's instructions, the tools must be completely dry before placing in the high-level disinfectant as the water can dilute the disinfectant.

    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Staff ran to back cleaning area to hide disposable tools. A foot scrub was found to be re-used and cleaned. These are one time use only and need to be discarded after one time use.
    • 20. Is reusable equipment disinfected properly?
      • The concentration of high-level disinfectant was not being adequately tested and tracked. - Ensure disinfecting solutions are being tested and values are recorded daily. Keep a record of the concentrations on-site.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 2) Wax pots had a build-up of old wax and debris from cotton swabs on them. - Ensure wax pots are being cleaned frequently.

      • The concentration of high-level disinfectant was not being adequately tested and tracked. - Ensure disinfecting solutions are being tested and values are recorded daily. Keep a record of the concentrations on-site.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 1) Surfaces throughout the facility were being disinfected with alcohol instead of a low-level disinfectant. - Acquire an approved low-level disinfectant such as Lysol to use for surfaces such as pedicure chairs, and waxing beds. 2) Wax pots had a build-up of old wax and debris from cotton swabs on them. - Ensure wax pots are being cleaned frequently.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Dirty mop water was stored in the reprocessing area. - Do not keep mop bucket in the reprocessing area as there is the potential for cross-contamination. Move the mop bucket into the washroom or empty the water from the bucket after each use.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Facility was mixing bleach and Mr. Clean together to create a solution to disinfect the pedicure basins.- Do not mix together disinfectants as they can negatively react with each other.

      • Used single-use items such as nail files, and sponges were found in drawers. Single-use spoons were found left inside of scrub products. - Ensure all products that are single-use are used once then thrown away.

      • The concentration of high-level disinfectant was not being adequately tested and tracked. - Ensure disinfecting solutions are being tested and values are recorded daily. Keep a record of the concentrations on-site.
