- 1) Bowls used as scoops were stored with in the dry storage containers.Action required: -Make sure that handled scoops are used for dry storage containers and must be stored outside in clean and sanitary manner.2) PHI observed that cooking, eating, storage and laundry intended for personal reasons was occurring in close proximity to the commercial food processing.Action required: -Operator must ensure that there is clear separation between area, for personal activities and commercial use.

- -One of the cooler close to the front cash counter was not working properly (measured at 16 degrees).-Operator was instructed to service that cooler and mean while use other coolers in the front entrance area.

- Chlorine test strips were not available.Action required: Operator must ensure that facility has sanitizer test strips available.
