      • Detailed facility-specific written procedures were not available at time of inspection describing relevant safe and sanitary practices including sink decontamination, skin cleansing, handling accidental skin cuts or punctures, how to operate and clean/disinfect a mobile sink, skin cleansing and antisepsis, and post-service care. The operator mentioned they were located in a digital folder that was not accessible on-site. PHI trainee offered written templates for these procedures onsite (except for post-service care), as well as education on how to fill out and maintain the procedures.Please ensure all written procedures are accessible on-site where personal services will be conducted, and please send the digital versions (if available) to this office via email for review as soon as possible.

      • Post-service care instructions for tattooing/micro-needling are provided verbally by the operator, but not in writing. Please ensure to provide written and verbal instructions for post-service care for personal services that puncture the skin.

      • Personal services workers provide client records, but they do not contain sterility documentation (needle Lot #) attached to each client record.Please ensure that client records contain sterility information (needle lot numbers) attached to each client's records, as well as their full name, contact information, date of service, description of service, and employee who performed the service.
    • 20. Le matériel réutilisable est-il correctement désinfecté?
      • High-level disinfectant was used (PREempt CS20) for disinfecting semi-critical instruments, including tweezers and comedone extractors (metal hook) used to remove ingrown hairs, however, the operator did not soak the tools fully, or for the contact time indicated by the manufacturer (20 minutes) due to concerns with tool corrosion. Testing strips and records for testing were not present onsite. High-level disinfectant was not reused. If contact time is not maintained and semi-critical tools are not fully soaked with disinfectant after a cleaning step, pathogens may not be sufficiently reduced to eliminate as much risk of skin infection as possible.Please switch to using disposable single-use tools for this purpose, or another high-level disinfectant that will not corrode tools. Make sure the high-level disinfectant used has a medical device license (MDL) number, test strips and a testing log are used daily, and to fully immerse metal semi-critical tools for the contact time required in the manufacturer's instructions for use for the specific high-level disinfectant used.
    • 30. La zone de retraitement d'une installation de services personnels qui utilise du matériel réutilisable est-elle bien conçue, gérée et entretenue pour assurer la sécurité des clients?
      • The reprocessing area for semi-critical tools (tweezers and commodone extractor) is in a sink in the same room that contains a toilet. PHI trainee explained to the operator that reprocessing areas must be protected from sources of contamination, and another permanently plumbed sink would be required for use. Please ensure that semi-critical tool reprocessing occurs in the other permanently plumbed sink located in the spray-tanning room, or another permanently plumbed sink that is not in the same room as a toilet.
