    • 9. WATER SOURCE: SAFE, HOT & COLD UNDER CITY PRESSURE
      • No hot running water under city pressure. Observed that establishment does not have hot running water at handsink for proper hand washing, utility sink or three compartment sink to properly wash, rinse or sanitize utensils or equipment. Temperature of hot water at 55.5F. Instructed that premises must have hot running water at all times per municipal code of Chicago. Critical citation issued 7-38-030.
    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • Evidence of rodent activity on premises. Observed over 50 mice dropping under display shelves on north and south walls. Instructed to remove all rodent activity, detail clean and sanitize areas. Must have licensed pest control service premises. Serious citation issued 7-38-020.

      • Dish washing facilities not maintained or operated. While draining water in middle of three compartment, water coming up left side is orange and greasy with food debris. Instructed to detail clean grease trap and drainline to prevent inadequate disposal water waste at sink. Serious citation issued 7-38-030.

      • Filters not installed at exhaust hood. Instructed to provide filters at exhaust hood.

      • Floors not maintained or cleaned through out. Instructed to detail clean all areas and replace any missing or damaged floor tile. Must reseal floor in rear near exit door.

      • Coving not installed along the wallbase through-out. Instructed to install coving and seal gaps between floor and wallboard. Ceiling throught-out not maintained. Instructed to replace any missing or damaged tile. Several holes and openings noted throught-out premises. Instructed to seal all holes throught-out including around pipes. Rear of premises not maintained. Instructed to replace or repair exising walls to make smooth, easily cleanable and light in color. Clean and maintain walls through-out premises.

      • Light bulbs not shielded in prep area. Instructed to install shield over bulbs.
    • 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
      • Exposed handsink next to three compartment sink not maintained. Instructed to install shield between both sinks. Drainline under three compartment sink leaking. Instructed to repair drainline to prevent leak. Ventilation not provided in toilet rooms. Instructed to install exhaust fan or have both windows opened and screened at all times of operation.
    • 40. REFRIGERATION AND METAL STEM THERMOMETERS PROVIDED AND CONSPICUOUS
      • Some coolers missing conspicuously placed thermometers. No metal stem thermometer. Must provide required thermometers.
    • 41. PREMISES MAINTAINED FREE OF LITTER, UNNECESSARY ARTICLES, CLEANING EQUIPMENT PROPERLY STORED
      • Unnecessary articles through-out premises. Instructed to remove all unused articles, bed and propane tank and elevate all supplies 6 inches or higher off floor. Remove milk crates and provide raised shelving. Remove utensils rack by exposed handsink to provide easier access to sink.
