- Written procedures were not established for decontamination of dual purpose sinks (i.e. used for hand washing and equipment decontamination). Written procedures also to be in place for procedures in the event of an accidental puncture wound or cut. Please develop written procedures as per the requirements of the Personal Services Standards.

- Testing strips being used do not read high enough for the Pre-empt CS20 (7% hydrogen peroxide). Please purchase test strips so can properly verify the concentration of high level disinfectant daily.
