    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • The operator has not notified the Alberta Health Services of the opening of a personal service business.Make sure that business provides notification to AHS about opening of a personal service business.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Facility-specific written procedures were not available at the time of inspection.Make sure that facility maintains the written procedures for safe and sanitary handling of equipment, surfaces and tools.
    • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • 1) -The inner lining of the washing machine was stained black; evidence of mold. 2)The changing slippers available for clients were dirty and need cleaning.Make sure that indicated areas are cleaned and maintained in clean and sanitary condition.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Single-use spatula sticks were observed to be re-used in the bulk wax products. Make sure that bulk products are dispensed using single use spatula sticks and discarded after each use.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Visibly dirty re-usable equipment like tweezers were observed to be stored with clean items in client service area.Make sure to properly clean and disinfect this equipment before re-using/storing.
    • 20. Is reusable equipment disinfected properly?
      • Facility had High level disinfectant available Pre-empt CS-20 at the time of inspection, but it was not used according to manufacturer's instructions. Pre-empt was mixed with blue soap beads for equipment disinfection.Test strips to check the High level Disinfects (HLD) were not available.Logs for HLD were not available.Make sure to follow the manufacturer instructions for contact time of HLD.Make sure that test strips and Logs for HLD are available.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1)During the inspection it was observed that baby powder and oil bottles were visibly dirty.2)Cosmetic products used for tinting eyebrows were labelled as "not to be used for eyelashes or eyebrows".3)Melted paraffin wax equipment in storage area was dirty and was dispensed using Styrofoam paper cup.Make sure that containers for cosmetic products are handled in clean and sanitary manner.Follow manufacturer's instructions for use of cosmetic products.Use handled scoop to dispense from bulk wax product to avoid cross ocntamination.
