      • Written Procedure for Handwashing and Equipment Reprocessing Sink Decontamination was not implemented, observed staff washed their hands at the tool reprocessing sink and dried their hands with a reused towel. - disinfect the tool reprocessing sink before tool reprocessing and equip hand drying suppliers (paper towel) OR designate the sink for tool only.

      • Single use wooden sticks were left inside the sugar scrub containers in the pedicure station. - Discard single use items after each use to avoid cross contamination.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • tweezers for eyebrow were found on the soap dispenser inside the treatment room. - Ensure tools are properly stored after cleaning and disinfection.
    • 20. Is reusable equipment disinfected properly?
      • Dirty semi-critical tools, such as nail clippers and cuticle cutter, were found on every manicure stations. - Remove all the dirty tools. Only cleaned and disinfected tools are allowed to be used. Preempt CS20 onsite was expired. - Obtain unexpired disinfectant.Cleaned tools were not properly dried before submerging into CS20. Disinfected tools were not properly rinsed before storing. - Follow manufacturer's instructions, education provided onsite. Recommend posting the instruction in a language that staff understands.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • All of the manicure workstations, including countertop, electrical filler body, tool holders, drawers, and the gel containers, were covered in dust.- Ensure all the stations were cleaned daily. Buffer and filer for fake nails were stored at workstations disorderly. - Ensure tools are organized and stored in labeled containers.

      • The lash glue label print was not legible to ensure manufacturer's instruction is properly followed.-Please provide and use lash glue with legible manufacturer's instructions and ingredients list.

Operator was not disinfecting reusable semi-critical tools such as nail cutter or cuticle remover with high-level disinfectant.-Please provide and use effective high-level disinfectant such as preempt CS20 to disinfect the indicated tools to prevent spread of infection.2. Alcohol was used to disinfect lash extension tweezers.-Please use high-level disinfectant such as preempt CS20 to ensure proper cleaning and disinfection of such tool.
