    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Upon discussion with owner, foam files were being reused on multiple clients as the owner was told by the supplier that they are cleanable. I advised the owner that despite what the supplier has informed them, as per the Personal Services Regulation and Standards in Alberta, these foam files could not be easily cleaned and disinfected, thus, shall not be used on more than one client.The owner was agreeable and will inform all staff that these foam files are single-use and must be discarded after use on each client.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • A couple pieces of equipment (manicure/pedicure tools) had visible debris. Advised owner to remove these and properly clean/disinfect. Also advised owner to inform/re-train all staff regarding proper cleaning and disinfection of equipment.Note:Owner noted she color coded some of the equipment to be able to keep track of which staff may be improperly cleaning/disinfecting the equipment.

    • 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
      • Written procedures for post-skin care and management of accidental cuts were not available. Additionally, the documented procedure for skin cleansing incorrectly referenced CS20. The operator was provided with the correct written procedures via email.
    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • The operator did not test the high-level disinfectant using test strips, and no documentation of test results was available for review.
    • 18. Is reusable equipment cleaned properly (Non-Critical)?
      • Tools were observed inside the UV sterilizer with visible debris. This indicates that the instruments were not properly cleaned prior to storage. The operator was advised to thoroughly clean all tools before placing them in the UV sterilizer to ensure effective infection prevention and control.
    • 20. Is reusable equipment disinfected properly?
      • The high-level disinfectant tray contained old, discolored liquid with significant residue buildup. No active disinfectant was present, and the tray showed evidence of not being cleaned or replaced daily as required.

      • The disinfectant PREempt CS20 was not tested for effectiveness using test strips, and the available test strips were found to be expired (EXP: 06/30/2025). The operator was instructed to test the disinfectant using valid test strips, and a documentation log was provided by the PHI both onsite and via email.

      • Several bottles were observed without labels. The operator was instructed to ensure that all bottles are properly labeled.

      • Several nail polishes were identified without ingredient lists or manufacturer instructions.All cosmetic products must be clearly labeled with a complete list of ingredients and appropriate usage guidelines.

    • 20. Is reusable equipment disinfected properly?
      • 1-The nail drill bits were reportedly disinfected in alcohol instead of high -level disinfectant. disinfect the used drill bits in high level disinfectant between client. Alcohol is not a disinfectant. All drill bits were removed from the station and moved to reprocessing area for cleaning and disinfecting using CS20. Operator will purchase an addition set to allow sufficient time for disinfection of used set between client.2-Bubbles were noted in high level disinfectant (HLD) container. Solution discarded and new solution was poured during inspection.Ensure the high-level disinfectant does not have bubbles. Dry the tools prior to soaking them in the HLD solution.3-The tools were not fully immersed in the CS20 high-level disinfectant. CS20 containing container was changed and tools were washed dried and immersed in the solution during inspection onsite.4-There was daily testing record of CS20 was available. The solution is reportedly changed everyday. The HLD testing log was available. New solution was tested and test results were entered in the sheet during inspection onsite.Ensure the CS20 is daily tested and test results are recorded on HLD testing log sheet and records are available in the facility for review during inspection.

      • Procedures were not being reviewed. Copies of the written procedures were provided to be reviewed.

      • Records of high level disinfectant were not maintained in the facility. HLD testing log sheet was provided onsite.
