      • Room A baby room: the low sink faucet was loose, the cupboard base beneath the sink was moldy/water damagedFix the faucet so that it is watertight. Remove the water damaged materials and replace with new materials. Room E: had water damaged materials under the sinks. Please ensure there is not plumbing leaks. Remove and replace the water damaged materials.

      • Blind cords were in reach in Room E, Room A, Room Please affix the blind cord up and out of reach of children Ensure that checking these cords is part of daily safety checks in the room.

      • Room A baby room: the low sink faucet was loose, the cupboard base beneath the sink was moldy/water damagedFix the faucet so that it is watertight.       • No available band-aids int the first aid kit for one of the rooms. Ensure all first aid supplies are monitored and available.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Drawer in disrepair. It appears that a work order is in place for this item. Please ensure all equipment is in good repair.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Paper towel was not adequately managed. Ensure adequate management of paper towel and ensure paper towel is protected from contamination.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • Safety lock was missing for the cupboard observed to be storing chemicals (i.e. soap, Windex) This was corrected by the operator (chemicals were removed). Art supply was observed to not be adequately managed. Paints with a no use sign for children 0-3 years old was observed in the infant room. Paint and tie dye supplies were observed to not have an AP od non-toxic sign on the containers. Ensure all art supply is adequately managed. A copy of the art supply guideline management will be sent with this e-mail.Electrical safety plug for an electrical plug was missing. This was immediately corrected by the staff member onsite.

      • Pool noodle covering faucet observed in one of the rooms. This appears to be done due to the water volume/splashes. Avoid this practice and ensure water and faucet is protected from contamination.A drawer full of sunscreen bottles was observed. Ensure all product is withing expiry date and accurately labeled. Several rooms have food stored in cupboards for storage of personal belongings/ chemicals. Some rooms have snack stored with chemicals next to it. Ensure snacks/food is adequately stored. Paper towel was not adequately managed. Ensure adequate management of paper towel and ensure paper towel is protected from contamination.

    • 26. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Critical)?
      • A severe rodent infestation was noted: mouse droppings within cabinet storage space under the classroom sinks throughout the daycare. The cupboard base openings around the plumbing had droppings visible within- Porte of entry. Mouse droppings noted direct on the floor adjacent to the hand sink cabinet Room A EHO discussed findings with the director who was already aware of the infestation and actively implementing various solutions. The cabinet floor pipe openings need to be sealed. These cabinets need to be emptied and monitored and droppings actively cleaned up to ensure a clean and Hygenic environment until the openings are sealed. Then ongoing pest control routines.

      • Several pillows, lounge chair cushions and small fabric couches had no covers. Ensure the item is easy to keep clean with a launderable cover(tight zippered cover is not easy to remove and clean)
