- Storage cupboards, located above client cubbies, in the Pine Room were disorganized and had a large amount of belongings not easily identified or grouped for what client they belonged to. Organization required, and/or sending items home that are not in regular usage.

- 05. Are communicable diseases at this facility being properly managed?
- Facility was keeping record of all clients who were ill; both those calling in sick and those who were sent home ill, as well as their symptoms. However, facility was not reporting/including cases where the client called in sick. These clients still count toward overall facility cases, and are also reportable if they have been in attendance the day prior to becoming ill. How, when and what to report was discussed with Assistant Director Sasha, and a page was printed from the Outbreak Guide to share with all staff.

- Facility's current illness policy at time of inspection stated clients could return if they were 24 hours symptom-free for vomiting and/or diarrhea. Clients and staff must be 48-hours free of vomiting/diarrhea symptoms before returning. Instructed Assistant Director Sasha to have the illness policy amended immediately and to communicate this change with all staff and parents.

- An educator was observed in the Pine Room washroom with three children; while diapering one, she finished with the child, set the child down and requested for the child to wash her own hands. the child cried and refused as the educator then disinfected the diapering surface. When the child repeatedly refused, the educator then picked up the next child and began diapering them. Neither the educator's hands nor the first child's hands were washed. Discussed these observations with Assistant Director Sasha.

- ***There was a roll of paper towels that were soiled present as the only hand drying option on top of the dispenser. She had a staff refill the dispenser and removed the roll. Paper towels are to be in dispensers as this helps ensure that they are clean and sanitary when used for drying hands.

- ***There were worn horizontal surfaces observed on the wooden furnishings (especially in Aspen room). The finishes had worn off and the surfaces were uncleanable.

- ***The hallway washroom did not have soap accessible to the children. The soap was out of reach on top of the paper towel dispenser.

- ***Aspen room had a toy car garage that appeared to be collapsing. Staff removed it during the inspection.***There was a build up of dirt on the yellow rubber bouncy dog and on the non brown areas of the moose. Discussed with staff and they cleaned right away.

- ***There was some debris present in some of the toy bins (discussed with staff in each of the rooms).

- ***There was a concern with the cross connection with the faucets in the two infant rooms. The faucet heads had extensions that created cross connections. The faucet extensions defeated the air gap requirement by dangling the end of the extension into the sinks. Discussed and one option is to remove the extensions or put in a different style of faucet (like a kitchen sink faucet with the retractable faucet head).

- ***the hot water temperature was >49C. The hot water is to be maintained between 38C and 43C.

- ***Discussed the emergency medication. It was stored at the bottom of the large pocket of the backpack. This does not make for timely access in an emergency. Discussed keeping it in an easily accessed outside pocket (ideally the same pocket for all the rooms to that staff will always know where it is even if they are not in their regular room). Many rooms had a second black bag dedicated for the emergency medication storage.Staff in the room moved the emergency medication to an outside pocket on the backpack.

- ***The wall mounted diaper changing surface closer to the Pine room was missing part of the belly strap. Staff in that room indicated that they try and use the other change surface closer to the Elm room.

- ***There was a build up of dirt present on the wooden stove unit in the Maple OSC room. The wooden burners were discoloured and there was build up on the lower doors of the unit.

- ***There were concerns with the sand play tables.-Aspen room had wet sand present after it was left overnight. Between the wetness and the potential bacteriological build up, this sand should not be used for children to play with.-The sand in the Maple room and in the Birch rooms were not dust free. (when a handful of sand was picked up and dropped, or when children were pouring sand from one container to another, there was a cloud of dust present. Staff had not opened the sand table in Maple and closed down that area in Birch during the inspection.

- ***There was a slide set up in the piazza area. The fall height was 24inches high from the top rung. Discussed that protective fall surfacing was required for vertical distances over 18 inches. If slide top can be set up to be lower than 18 inches then no fall surfacing is required.

- The water temperature at one of the hand washing faucet was measured at between 10 to 11 degrees C.Water temperature at other faucets was appropriate between (38 degrees C to 43 degrees C).-Discussed with facility director, she informed PHI that hot valve was turned off for re-filling water bottles.-Hot water valve was turned on, temperature was re-measured at maximum of 30 degrees C.-Make sure that water at hand washing station is maintained at proper temperature to encourage hand washing.

- *The blind controls were discussed and demonstrated how they were too slack and could pose an entrapment hazard. It was reported that they had been raised and were supposed to be out of reach from the children. They still would remove links to shorten the chains.

- Knives and scissors were stored with clients reach. Two wood logs with multiple nails were nails were noted in the 4 years old classroom. The operator indicated that this age group uses nails and hammer to make these logs. Please be advised that all sharps must be stored away from clients reach.

- A safety belt was not observed at the diaper changing station of the baby room. Please install the missing safety belt.

- Boxes of gloves and a towel were noted to be stored on top of garbage containers. Garbage containers were noted to be full. Please be advised that garbage containers must not be used for storage purposes. All garbage containers must remain clean and accessible. Please change bags as needed.

- General cleaning and organizing is required throughout the facility. Used towels were noted to be on the floor. Please organize the facility thoroughly and ensure it is being maintained in a clean and sanitary manner.

- Bite marks were noted on multiple foam blocks throughout the facility. Please remove all blocks with bite marks.

- 1. Sleeping supplies (bedding) were not being stored properly. Please be advised that all sleeping supplies must be stored in individually labelled bags/container. 2. Clients in kinder room were noted to be in contact with other clients blankets. Please be advised to acquire sleeping supplies and store them properly if this area is being used for napping purposes.

- Use of sanitizer: There was a bottle with weak concentration (Staff reported that it may have been from yesterday) was full indicating that the sanitizer may not be being used, The sanitizer bottle in the OSC/kinder room was full after being used for snack).Ensure sanitizer is being used at appropriate times.

Open magnets are magnets in toys that are not completely enclosed and may fall out or be removed and potentially swallowed. Ensure manufacturers instruction with regards to age appropriateness are followed.
