- Many small drain flies noted in the bar area. - Please consult with pest control regarding the flies, remove any standing water/moisture from this area.
- 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
- Flooring in back "preparation room" is carpet. Is very dirty and carpet cannot be cleaned properly.- Please replace the flooring in this area, change to a material that is smooth and cleanable.

- Chemical spray bottles were mislabeled. QUAT sanitizer was labelled "glass" and other sanitizer bottle was unlabeled. - Labels were changed onsite, please ensure any chemicals used are adequately labelled to indicate their contents.-
- 10. Are food temperatures during storage and processing being controlled to maximize food safety?
- Storage fridge in the back preparation area was warm. Measuring at 47F. - Do not use this fridge until it can hold temperatures below 4C. Repair the fridge.
- 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
- Glasswasher was initially out of iodine. Operator was able to replace onsite. - Please ensure chemicals are stocked and glasswasher can adequately sanitizer glasses. Ensure chemical concentrations are tracked.
- 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
- Hand sink was initially blocked by dirty dish cart. - Cart was moved, please ensure hand sink is accessible to staff at all times.
