- A square handleless container was being used as a bulk scoop. Please ensure that all bulk scoops are equipped with a handle to protect the food from contamination.
- 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
- Hand washing sinks are being used as dump sinks at the bar. Please ensure that hand washing sinks are designated for hand washing only. The facility is equipped with two hand washing sinks and two dump sinks at the bar. Please only use each sink for its intended purpose.
- 15. Is the facility free of a pest infestation?
- (Outstanding violation). There are numerous flies observed throughout the main service area of the facility. The facility must be free from a pest infestation. Consult with a pest control professional and address the issue accordingly. In the meantime, perform enhanced cleaning to remove sites of breeding, food sources and ensure food are covered when not in use to prevent the attraction of flies.

- 1) Pest control service reports indicate mouse activity in the dining room. No droppings were observed in any of the kitchens/bar. Regular service from the pest control company is ongoing to abate the pest issue. 2) Two German cockroaches were observed on a monitoring glue board in the empty recyclable bottle room in the basement.Treatment for cockroaches was noted on a pest control report. No other signs of cockroach activity were observed. Please work with the contracted pest control company to abate all pest issues.

- A build-up of dust and debris was observed in the cupboard in the polishing area. Please clean.

- Several spatulas found in the storage area were in disrepair. Utensils must be maintained in good repair to prevent physical contamination of food. Discard utensils when they are worn out. The operator discarded the utensils at the time of the inspection.
