      • Written procedures were not readily available for review for cleansing skin and accidental skin cuts and punctures.Ensure that the written procedure documents for services are kept on site at all times. An operator of a Personal Services Establishment must be able to easily access these documents for review/revision and when requested by a Public Health Inspector.**Update (February 6, 2026): Operator submitted written procedures for Cleansing of the Skin for Waxing Services and Accidental Skin Cuts and Punctures. Procedure for Cleansing of the Skin for Brown and Lash Services still need to be completed.

      • Written procedures were not readily available for review for cleansing skin and accidental skin cuts and punctures.Ensure that the written procedure documents for services are kept on site at all times. An operator of a Personal Services Establishment must be able to easily access these documents for review/revision and when requested by a Public Health Inspector.
    • 20. Is reusable equipment disinfected properly?
      • Daily testing results for high-level disinfectant (CS20 - MDL 193055) are inadequately maintained. A disinfectant log sheet was provided to operator during the inspection. Daily test results of high-level disinfectants must be monitored, recorded and maintained to ensure that the minimum effective concentration for disinfection is verified.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Two bottles containing liquids were observed to be unlabeled on the counter next to the wax pots. Operator labeled both bottles with a permanent marker during the inspection. All cosmetic substances and similar agents stored in containers must be adequately labeled to easily identify their contents and prevent accidental misuse.

      • The following item was noted to be expired: 1. PREempt CS20 7% Accelerated Hydrogen Peroxide test strips (Exp: April 2024) Please ensure new test strips are obtained and available for accurate concentration testing/monitoring of chemical disinfectants.
