- The operator failed to disinfect the dual-purpose sink after tool reprocessing, only performing a cleaning with soapy water. Disinfection using PREempt RTU is now mandated for the sink following all tool reprocessing procedures.

- Log Review Findings: High-Level Disinfectant (CS20) Protocol- Non-Compliance: Facility logs indicated that the poured CS20 solution was maintained for 16 days before disposal, two days beyond the 14-day reuse limit.- Root Cause: The operator incorrectly based the 14-day reuse period solely on business operating days.CORRECTION: The operator was instructed that the poured solution must be discarded after a maximum of 14 calendar days (from pour date to discard date), irrespective of whether the facility is open or the solution has been used.

- Used single-use nail files, buffers and gloves were observed at working stations. The staff present discarded the items during inspection.Please ensure used single-use items are discarded immediately after use and not re-used.
- 20. Is reusable equipment disinfected properly?
- High-level disinfectant testing log was not being maintained. A copy of the log sheet was given to the operator during inspection. Education on how to test high-level disinfectant and log sheet recording was provided during inspection. Poured high level disinfectant used at the facility must be tested each day used and record kept. When testing fails, the solution must be discarded. Poured solution should also be changed after 14days.
