- Reusable cleaned tweezers were noted being stored inappropriately on the cart at eyelash extension station. Tools were stored on top of open cart in unsanitary manner. Discussed with operator proper storage of reprocessed tools and equipment. Store in clean containers that protect tools from contamination.Ensure clean tools are stored in a way to prevent their contamination.

Obtain reusable containers to pre-portion the cosmetic products and keep the bulk supply covered to prevent contamination during storage. -Recommendation only: RefectoCil tinting products were not labeled with beyond use date. Recommended to operator to label tinting products with their beyond use date.

- Alcohol based hand rub (ABHR) was noted without DIN or NPN at front service desk. The hand sanitizer was discarded during inspection onsite. A hand sanitizer with NPN number was provided at the desk during inspection onsite.

- Used linen on spa bed for eyelash extensions. Advised operator linen to be changed and laundered in between clients. Operator removed linen to be laundered. As spa bed surface material impervious to moisture operator may use disposable barrier already available and discard between clients then clean spa bed surface.

- 1. Low level disinfectant (LLD) Mr. Clean M. Net reportedly being diluted by mixing with water and soap prior to use on pedicure basins. Manufacturer instructions for use reviewed with operator.Ensure the disinfectants are used according to manufacturer's instructions for use. Intermediate level disinfectant (ILD) recommended to operator for pedicure basin disinfection, but noted LLD minimum requirement for non-critical equipment. 2. Semi critical tools being reprocessed with high level disinfectant (HLD) were noted not being fully immersed in solution. Reviewed with operator requirement for tools and equipment to be completely immersed for adequate disinfection. Tools to be reprocessed. Obtain an appropriate container/dish with a wider based to ensure the tools are fully immerse in HLD during reprocessing. 3. Noted container of PREempt Concentrate ILD expired 09/2022 and PREempt CS20 HLD expired 05/2023. As per operator these products are being used for equipment disinfection. Operator obtained new containers of PREempt CS20 and PREempt RTU during inspection. Existing poured disinfectant was discarded and operator instructed to discard bulk expired disinfectant. 4.Daily testing of HLD not being completed or recorded. 5-Virox test strips for PREempt CS20 expired 09/2022. Operator obtained HDL Virox test strips with valid expiry date during inspection. New HLD solution poured and tested as per manufacturer's instructions by operator. AHS HLD log sheet provided and completed with operator. Operator aware to record daily testing of HLD including date of test, results, when disinfectant is changed, and initials of person performing test. 6. Metal drill bits stored in solution in containers on manicure stations with liquid indicated to be alcohol solution by personal service worker. Advised that semi-critical equipment to be cleaned and disinfected with HDL per manufacturer's instructions.

- The sharps were noted being dispose off in an open plastic coffee mate container store under reprocessing sink. There was no sharp container in the facility.Obtain a puncture resistant sharps container with tight fitting lid and safely dispose of sharps via approved sharps disposal service or location such as pharmacy.

- Observed reprocessing sink being used for handwashing. Advised operator reprocessing sink must not be used for handwashing unless written decontamination procedure completed. Operator agreeable to use washroom sink for handwashing and designate a separate sink for reprocessing only. Post signage stating "No Handwashing and Reprocessing Sink only" at reprocessing sink.
