- 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
- Eyelash extension tools/instruments were observed on-site in the waxing room. PHI was advised that this service is not offered to the public. Please remove all eyelash extension tools/instruments from the facility.Re-iterated the requirement to notify AHS of new services.
- 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
- A client was observed to be washing their hands at the reprocessing sink which is not permitted. According to the written procedures posted on-site, clients should wash their hands in the bathroom sink.Staff are not following manufacturer's instructions for high-level disinfectant use. Staff must wash tools with soap/water, dry and fully soak tools in high-level disinfectant for time indicated on the label. Please re-iterate these requirements with all staff.
- 13. Is single-use or uncleanable equipment being used as intended (Critical)?
- Single-use items (i.e. cardboard nail files, cardboard sleeves for rotary nail drill etc.) were in a used condition at the manicure stations. These items are single-use and must be discarded after each use. Please re-iterate this requirement with staff.
- 20. Is reusable equipment disinfected properly?
- 1. Metal rotary drill bits were being disinfected in isopropyl alcohol which is not considered to be an adequate disinfectant. The metal rotary drill bits must be disinfected in high-level disinfectant. Please re-iterate this requirement with staff.2. The high-level disinfectant tray did not contain enough disinfectant to fully soak tools. Ensure tools are being fully soaked as required.3. High-level disinfectant logs are not being properly maintained. Re-iterated the requirements on how to properly maintain HLD logs. Please re-iterate this requirement with all staff.4. The AHP7 indicator test strips have expired. These strips have a shelf life of 3-months once opened. Obtain new AHP7 indicator test strips.5. Tools are not being soaked in the high-level disinfectant for the time indicated on the label (20 minutes). Please re-iterate this with all staff.

- There were unlabeled cosmetic products in various areas. Label all items/bottles/containers etc. with its contents throughout the facility and ensure original/bulk-supply containers are kept on-site.
- 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
- A client was observed to be washing their hands at the reprocessing sink which is not permitted. Please advise all staff to have clients wash their hands at the bathroom sink.
