      • - Pre-empt CS20 (a high-level disinfectant) was available - High level disinfectants are not being tested and verified daily using proper test strips. Test strips to verify the concentration of 7% of hydrogen peroxide (Pre-empt CS20) were not available.- Disinfection records were available. HLD log was reviewed with the operator.

      • Written procedures for client care in an event of accidental cut and puncture and cleansing of skin were not available.The operator was advised to develop the written procedures.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • - Semi-critical tools (2 nail clippers and a nail nipper) stored with clean equipment were observed to be contaminated at the point-of-use. Semi critical tools were not being adequately cleaned and dried prior to disinfection or sterilization. Ensure equipment is: disassembled, cleaned to remove soil, and rinsed and dried prior to disinfection or sterilization.The operator took all the soiled tools to the reprocessing area on PHI's request.Ensure clean equipment is visually inspected to be clean at the point-of-use.
    • 20. Is reusable equipment disinfected properly?
      • - Pre-empt CS20 (a high-level disinfectant) was available but was expired.Semi-critical equipment is not being adequately cleaned and disinfected. Ensure semi-critical must be cleaned and disinfected using, at a minimum, a high-level disinfectant.- High level disinfectants are not being tested and verified daily using proper test strips. Test strips to verify the concentration of 7% of hydrogen peroxide (Pre-empt CS20) were available but were expired in the year 2024.Ensure the high-level disinfectant is tested daily using the test strips specified by the disinfectant manufacturer.- Disinfection records were not available. Ensure records are kept of: test dates, whether the solution passed/failed, when the disinfectant was changed and the worker’s initials who performed the activity.

      • -The grated callus remover was not being completely immersed in the disinfectant to enable proper disinfection. The operator was advised to acquire a bigger container to completely immerse the semi-critical tool in the HLD.Ensure equipment is fully immersed in solution.- Pre-empt CS20 expired in the year 2021 was the only HLD available in the facility. The operator informed that they mix water in Pre-empt CS20 and use the prepared solution to disinfect hard and non-porous surfaces.Personal services workers are not following all of the disinfectant manufacturer's instructions for use. Ensure manufacturer’s instructions are being followed.
    • 20. Is reusable equipment disinfected properly?
      • -No disinfectant was available to disinfect hard and non-porous surfaces.-Alcohol was being used to disinfect non-critical equipment.-The operator was advised to acquire, at a minimum, a low-level disinfectant to disinfect hard and non-porous surfaces as well as all non-critical equipment.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Pedicure scrubs were being portioned using steel spoons, which were stored inside the scrub.This could lead to potential contamination of the pedicure scrub by repeated contact between contaminated spoon and the pedicure scrub. The operator was advised to either use pre-portioned pedicure scrubs and masks in small single-use plastic containers or single-use spatula/implements to portion the pedicure scrub and mask.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • -The reprocessing area did not have adequate space to maintain one-way flow of tools.The operator was advised to declutter the shelves beside the reprocessing sink to make adequate space for one-way flow of tools.

      • High Level Disinfectant Logs were not being completed at the time of Inspection.Ensure that High Level Disinfectant Test Strips are being kept on site in order to confirm the concentration of Disinfectant being used. This information should be recorded daily in the High Level Disinfectant Log.This will help to ensure that high risk tools are being Disinfected adequately. This was Corrected during the Inspection.High Level Disinfectant(CS20) and Test Strips were available on site at the time of Inspection.
