      • Open wall area with exposed insulation was observed behind both sides of the line coolers in the bar section.- Refinish the wall and seal the gaps to ensure surfaces are smooth, cleanable, durable and impervious to moisture.

      • Mouse droppings were noted behind the ice machine and under the seats in the customer dine-in area.** Operator is to ensure to remove droppings using appropriate methods and thoroughly clean and sanitize all the areas.

      • Diatomaceous earth (DE) dust was observed applied in the kitchen and surrounding areas, and pest traps were affixed to the wall above the dessert cooler near the stairs. **Operator is to ensure that diatomaceous earth dust and all pest control products are used only in accordance with manufacturer instructions and applied in a manner that prevents contamination of food and food‑contact surfaces.**Ensure diatomaceous earth dust is not applied during food preparation or operational hours when food, equipment, or utensils are exposed.

Significant accumulation of grime and debris was observed throughout the facility, in areas including but not limited to: - underneath and behind the dishwashing area, - underneath and behind the ice machine, - on the wall besides the ice machine,- underneath the stairs, - behind the line coolers in the prep kitchen, - and in corners/hard-to-reach areas throughout the facility. **Please thoroughly clean all the aforementioned areas.2. There was no sanitation plan or cleaning schedule available for the facility.**Develop and implement a written sanitation plan.
